Association Between Family Functioning and Smoking Cessation Status among Smokers in Outpatient Department, Phramongkutklao Hospital

Abstract

          This cross-sectional study compared family functioning between current smokers and individuals who had successfully quit, and explored differences in smoking characteristics and perceived family attitudes toward smoking. Fifty-one participants were enrolled: 27 current smokers and 24 quitters (19 remote, 5 recent). Sociodemographic data, smoking history, family attitudes, and family functioning (Chulalongkorn Family Inventory [CFI]) were collected via structured questionnaire. Categorical variables were analyzed with Chi-square tests, continuous variables with Mann–Whitney U tests, and logistic regression was used to assess associations between CFI scores and smoking cessation status.

            Quitters were significantly older, more often married, more highly educated, and more likely to work outside the government sector than current smokers (all p < 0.05). Smoking-related characteristics and perceived family attitudes toward smoking did not differ significantly between groups. Overall family functioning was good (median total CFI 108, IQR 101–118). Quitters had higher CFI Problem Solving scores (20 [18–21] vs 17 [15–20], p = 0.021) and Role scores (11 [10–11] vs 10 [8–11], p = 0.014). Logistic regression indicated increased odds of quitting per one-point increase in Problem Solving (OR 1.27, 95% CI 1.037–1.556, p = 0.021) and Role (OR 1.748, 95% CI 1.121–2.724, p = 0.014). These findings suggest that older age, being married, higher education, non-government employment, and stronger family functioning—particularly in problem solving and role performance—are associated with successful smoking cessation and may inform the design of cessation programs.
