Effectiveness of self-management program by knee exercises in combination with hot compress in elderly with knees osteoarthritis in Primary Care Unit, Hang Chat Hospital, Lampang Province

Abstract
Background: Elderly with knee osteoarthritis often presents knee pain and leg muscle weakness. Patients can self-care at home to reduce the severity and increase their ability to perform daily activities. This self-management program may help the elderly with knee osteoarthritis in the community continually self-care and reduce knee osteoarthritis ’s impact.
Objective: To examine the effectiveness of self-management program by knee exercises with hot compression toward knee pain, functional leg strength, and balance in the elderly with knee osteoarthritis.
Methods: This quasi-experimental was conducted among 60 elderly who were diagnosed with knee osteoarthritis in the Primary Care Unit, Hang Chat Hospital, Lampang Province. Thirty elderlies in the experimental group received a self-management program by knee exercises with hot compression for 8 weeks-5 days a week. This program also provided a hot pack, manuals of knees exercise, and activity notes. Thirty elderly people in the comparison group received routine healthcare services. Knee pain was assessed using the modified WOMAC Thai version, functional leg strength was assessed using 30 seconds chair stand, and balance was assessed using the Berg Balance Test. Data were analyzed using independent t-test, paired t-test, and multivariable Gaussian regression.
Results: After receiving self-management program, experimental group had lower knee pain scores, higher functional leg strength, and higher balance scores. However, no difference was found between comparison and experimental group. After controlled confounding variables, only knee pain was significantly reduced.
Conclusion: Physiotherapists should implement this self-management program to reduce knee pain in the elderly with knee osteoarthritis.
