The Development and Evaluation of Perioperative Nursing Model for Hip Arthroplasty at Sakon Nakhon Hospital

Abstract

An effective and appropriate perioperative nursing model for hip arthroplasty enhances the quality of care. This research and development study aimed to explore the current situation, develop, and evaluate the outcomes of an operating room nursing care model. The study was conducted in four phases: 1) model development, 2) pilot implementation, 3) model refinement, and 4) outcome evaluation. The sample was selected through purposive sampling and consisted of 30 registered nurses and 30 patients. The research instruments included nursing practice guidelines, evaluation forms for nursing knowledge, skills, and competencies, a nursing supervision form, a focus group discussion guide, a patient data collection form, and nurse and patient satisfaction evaluation forms. Data were analyzed using content analysis, descriptive statistics, and inferential statistics, including the Chi-square test and the Wilcoxon Signed-Ranks test. The results revealed that: 1. The nursing model comprised five components: (1) clinical practice guidelines, (2) knowledge and skills, (3) nursing competencies, (4) coaching-based supervision, and (5) patient self-care guidelines. 2. Regarding patient outcomes, following the implementation of the nursing care model, patient identification accuracy and the preparation of unready surgical patients showed statistically significant improvements, whereas intraoperative blood loss showed no statistically significant difference. In addition, it was found that overall patient satisfaction was at the highest level. Concerning nursing outcomes, nurses' knowledge, skills, and competencies showed statistically significant improvements. Moreover, the results showed that compliance with the nursing care model during supervision was at the highest level (92.97%), and overall nurse satisfaction was also at the highest level. Therefore, this nursing care model should be continuously implemented to enhance the quality of nursing practice and promote positive patient outcomes.
