การพัฒนารูปแบบการจัดการภาวะน้ำตาลในเลือดต่ำ สำหรับผู้ป่วยเบาหวานโรงพยาบาลมหาราชนครศรีธรรมราช*

Abstract

          การวิจัยครั้งนี้ มีวัตถุประสงค์เพื่อพัฒนารูปแบบและศึกษาผลของการนำรูปแบบการจัดการภาวะน้ำตาลในเลือดต่ำสำหรับผู้ป่วยเบาหวานไปใช้ โดยประยุกต์ใช้กรอบแนวคิด CURN model ประกอบด้วย 4 ระยะ คือ 1) การวิเคราะห์ปัญหาและสถานการณ์ 2) การพัฒนารูปแบบการจัดการภาวะน้ำตาลในเลือดต่ำ 3) การนำร่อง/ทดลองปฏิบัติ และ 4) การปฏิบัติและประเมินผลลัพธ์

       กลุ่มผู้ให้ข้อมูลคือ ทีมสหวิชาชีพ 5 คน พยาบาลวิชาชีพ 45 คน และผู้ป่วยเบาหวาน 78 คน เก็บรวบรวมข้อมูล ณ.หอผู้ป่วยอายุรกรรม 4-7 โรงพยาบาลมหาราชนครศรีธรรมราช ระหว่างเดือนเมษายน 2558 ถึงเดือนตุลาคม 2558 วิเคราะห์ข้อมูลเชิงปริมาณโดยสถิติเชิงพรรณนา และข้อมูลเชิงคุณภาพโดยวิเคราะห์ เชิงเนื้อหา

            ผลพบว่า รูปแบบการจัดการภาวะน้ำตาลในเลือดต่ำ สำหรับผู้ป่วยเบาหวาน ประกอบด้วย 4 องค์ประกอบ คือ 1) การประเมินอาการ อาการแสดงและระดับความรู้สึกตัว 2) แนวทางการจัดการภาวะน้ำตาลในเลือดต่ำ แบ่งเป็น 3 กลุ่ม 3) การติดตามและประเมินผล ในเวลา 15 นาที และ 4) การประเมินปัจจัยเสี่ยงและการป้องกันการกลับเป็นซ้ำ ผลการนำไปใช้ ด้านผู้ป่วยพบว่าผู้ป่วยทั้ง 3 กลุ่มมีค่าระดับน้ำตาลในเลือดหลังการแก้ไขอยู่ในระดับที่เหมาะสม (80-180 มิลลิกรัมต่อเดซิลิตร) โดยมีค่าเฉลี่ยเท่ากับ 119.2 มิลลิกรัมต่อเดซิลิตร (SD=13.5) ด้านผู้ให้บริการพบว่าพยาบาลส่วนใหญ่แสดงความคิดเห็นต่อการนำแนวทางการดูแลไปใช้ว่ามีความเหมาะสม ง่าย ชัดเจน ทำให้เกิดผลดีต่อผู้ป่วย และทุกรายเห็นว่ามีความประหยัด สรุปได้ว่า รูปแบบการจัดการภาวะน้ำตาลในเลือดต่ำสำหรับผู้ป่วยเบาหวานตามแนวทางการศึกษาวิจัยในครั้งนี้ สามารถแก้ไขภาวะน้ำตาลในเลือดต่ำได้และทีมผู้ให้การดูแลผู้ป่วยได้มีแนวปฏิบัติที่ดี มีมาตรฐานเดียวกันในการแก้ปัญหาดังกล่าว อันจะส่งผลต่อการเพิ่มคุณภาพชีวิตที่ดีของผู้ป่วยเบาหวานต่อไป

Abstract

This research study aimed to develop a hypoglycemia management model for diabetes patients and also to evaluate the model effectiveness regarding how it can be applied to patients and healthcare professional. The CURN model was used as a framework for this study. Four phases were used for development of the model: 1) Problem and situation analysis, 2) Development of a hypoglycemia management model, 3) Testing the model, and 4) Applying and evaluating the model. Within the evaluation process, stakeholders who were informants consisted of 5 people from the multidisciplinary team, 45 registered nurses and 78 diabetes patients. All data were collected from medical ward (4-7) at Maharaj Nahkon Si Thammart hospital between April 2015 and October 2015. The quantitative data were analyzed using descriptive statistics and the qualitative data were analyzed using content analysis.

       The study reported that the hypoglycemia management model consisted of 4 elements: 1) Symptom and level of consciousness assessment, 2) Guidelines for hypoglycemia management, which was divided into 3 groups, 3) Monitoring and evaluation of guideline effectiveness within 15-20 minutes, and 4) Risk factors assessment and recurrence prevention. After applying the model into practice, it was found that all 3 groups of patients got an appropriate blood sugar level (80 -180 mg/dl) (= 119.2 mg/dl, SD = 13.5). Furthermore, the registered nurse who adopt the model into practice reported that the model was easily and functionally applied to nursing practice. Therefore, using the hypoglycemia management model seems to be possible because it is beneficial to patients and is cost effective. According to the study results, it could be concluded that the hypoglycemia management model is a realistic model for resolving hypoglycemia among diabetes patients. Moreover, it could become a practical guideline for healthcare professionals such that they can give diabetes patients standard care as well as enhance patient’s quality of life.
