บทคัดย่อ
อุบัติการณ์ของภาวะน้ำหนักเกินและอ้วนในสตรีตั้งครรภ์มีแนวโน้มเพิ่มมากขึ้น ภาวะน้ำหนักเกินและอ้วนเป็นสาเหตุของภาวะแทรกซ้อนในขณะตั้งครรภ์ คลอด และหลังคลอด สัมพันธ์กับการเจ็บป่วยและเสียชีวิตของมารดา-ทารก รวมทั้งปัญหาสุขภาพเด็กในระยะยาว ถึงแม้ว่าในระบบบริการสุขภาพจะมีการดูแลสตรีตั้งครรภ์ที่มีภาวะน้ำหนักเกินและอ้วนเพื่อป้องกันภาวะแทรกซ้อนในขณะตั้งครรภ์ แต่กลับพบว่ายังไม่ประสบความสำเร็จ เนื่องจากสตรีตั้งครรภ์รับรู้ว่าภาวะอ้วนไม่เป็นอันตรายต่อการตั้งครรภ์ ประกอบกับองค์ความรู้ของพยาบาลผดุงครรภ์และแนวทางการดูแลสตรีตั้งครรภ์ที่มีภาวะอ้วนที่ยังมีข้อจำกัดและจำเป็นต้องพัฒนาเพิ่มเติม ซึ่งเป็นความท้าทายของงานอนามัยแม่และเด็กในการออกแบบการบริการสุขภาพสำหรับการดูแลสตรีตั้งครรภ์ที่มีภาวะน้ำหนักเกินและอ้วน ปัจจุบันมีการนำหลักฐานเชิงประจักษ์มาพัฒนาเป็นแนวทางการดูแลสตรีที่มีภาวะน้ำหนักเกินและอ้วนตั้งแต่ระยะก่อนตั้งครรภ์จนถึงหลังคลอด เพื่อลดการเกิดภาวะแทรกซ้อนของมารดาและทารก และส่งเสริมสุขภาพสตรีโดยมุ่งเน้นการปรับเปลี่ยนพฤติกรรมสุขภาพ
