การพัฒนารูปแบบการดูแลสุขภาพของผู้ป่วยโรคเบาหวานชนิดที่ 2 โดยประยุกต์ใช้ทฤษฎีการจัดการตนเองแบบรายบุคคลและครอบครัว: การวิจัยและพัฒนา

บทคัดย่อ

โรคเบาหวานเป็นปัญหาทางสาธารณสุขทั่วโลกและมีแนวโน้มเพิ่มขึ้นอย่างต่อเนื่อง การศึกษาครั้งนี้เป็นการวิจัยและพัฒนา มีวัตถุประสงค์เพื่อพัฒนารูปแบบการดูแลสุขภาพของผู้ป่วยโรคเบาหวานชนิดที่ 2 โดยประยุกต์ใช้ทฤษฎีการจัดการตนเองแบบรายบุคคลและครอบครัว และศึกษาผลลัพธ์ทางคลินิกและผลลัพธ์ของโรงพยาบาล กลุ่มตัวอย่าง คือ ผู้ป่วยเบาหวานชนิดที่ 2 ซึ่งมารับบริการในคลินิกเบาหวาน โรงพยาบาลสมเด็จพระยุพราชเดชอุดม จำนวน 56 คน โดยเลือกแบบเจาะจง แบ่งออกเป็นกลุ่มทดลอง จำนวน 28 คน ที่ได้รับรูปแบบการดูแลสุขภาพของผู้ป่วยโรคเบาหวานชนิดที่ 2 และกลุ่มควบคุม จำนวน 28 คน ที่ได้รับการพยาบาลแบบมาตรฐาน เครื่องมือที่ใช้ในการวิจัย คือ รูปแบบการดูแลสุขภาพของผู้ป่วยโรคเบาหวานชนิดที่ 2 เก็บรวบรวมข้อมูลด้วยแบบสอบถามข้อมูลส่วนบุคคล แบบสอบถามพฤติกรรมการดูแลตนเอง และแบบบันทึกผลลัพธ์ วิเคราะห์ข้อมูลด้วยสถิติบรรยาย สถิติทดสอบทีแบบอิสระต่อกัน และวิเคราะห์ความแปรปรวนร่วม ผลการวิจัย พบว่า 1) รูปแบบการดูแลสุขภาพของผู้ป่วยโรคเบาหวานชนิดที่ 2 โดยประยุกต์ใช้ทฤษฎีการจัดการตนเองแบบรายบุคคลและครอบครัว มีดัชนีความตรงเชิงเนื้อหา อยู่ระหว่าง .85-1.00 มีความเหมาะสมและสามารถนำไปใช้ทางคลินิกจนเกิดผลลัพธ์ตามที่คาดหวัง 2) พฤติกรรมการดูแลตนเองโดยรวมของกลุ่มทดลองสูงกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ เมื่อมีการควบคุมพฤติกรรมการดูแลตนเองโดยรวมก่อนการทดลอง และ 3) ระดับน้ำตาลสะสมในเลือดเฉลี่ยของกลุ่มทดลอง (Mean=9.89, SD=1.69) ต่ำกว่ากลุ่มควบคุม (Mean=11.32, SD=1.86) อย่างมีนัยสำคัญทางสถิติ และค่ารักษาพยาบาลของกลุ่มทดลอง (Mean=413.39, SD=109.45) น้อยกว่ากลุ่มควบคุม (Mean=492.54, SD=186.78) อย่างมีนัยสำคัญทางสถิติ ดังนั้น จึงสรุปได้ว่ารูปแบบการดูแลสุขภาพของผู้ป่วยโรคเบาหวานชนิดที่ 2 ที่พัฒนาขึ้นในการศึกษาครั้งนี้ มีผลลัพธ์ทางคลินิกและอาจนำไปใช้กับโรงพยาบาลที่มีบริบทคล้ายคลึงกับโรงพยาบาลสมเด็จพระยุพราชเดชอุดมเพื่อช่วยลดค่ารักษาพยาบาลในผู้ป่วยกลุ่มนี้ด้วย
