Effects of an Oral Care Program on Oral Care Ability and Oral Mucositis among Cancer Patients Receiving Chemotherapy

Abstract

การวิจัยครั้งนี้เป็นการวิจัยกึ่งทดลอง (Quasi–experimental research) มีวัตถุประสงค์เพื่อศึกษาผลของโปรแกรมการดูแลช่องปากต่อความสามารถในการดูแลช่องปากและระดับความรุนแรงของภาวะเยื่อบุช่องปากอักเสบในผู้ป่วยมะเร็งที่ได้รับยาเคมีบำบัด กลุ่มตัวอย่างเป็นผู้ป่วยมะเร็งลำไส้และมะเร็งเต้านมเลือกอย่างเฉพาะเจาะจง จำนวน 15 คน ได้รับโปรแกรมการดูแลช่องปากที่ผู้วิจัยพัฒนาขึ้นโดยใช้ระบบการพยาบาลแบบสนับสนุนและให้ความรู้ร่วมกับแนวปฏิบัติการดูแลช่องปากของ BC Cancer Agency ซึ่งประกอบด้วย การสอน การชี้แนะและให้แนวทาง การสนับสนุน และการสร้างสิ่งแวดล้อมที่เหมาะสม ผู้วิจัยดำเนินโปรแกรมในระยะที่ผู้ป่วยได้รับยาเคมีบำบัด 3 ครั้ง (cycles) ต่อเนื่องกัน เก็บข้อมูลก่อนและหลังดำเนินโปรแกรม โดยใช้แบบประเมินความสามารถในการดูแลช่องปาก และแบบบันทึกภาวะเยื่อบุช่องปากอักเสบ เครื่องมือได้ผ่านการตรวจสอบความตรงเชิงเนื้อหา จากผู้ทรงคุณวุฒิ 3 ท่าน แบบประเมินความสามารถในการดูแลช่องปากมีค่า CVI =.90 และค่าสัมประสิทธิอัลฟาของครอนบาค =.91 วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา Friedman test และ Wilcoxon signed rank test 

          ผลการวิจัยพบว่า หลังเข้าร่วมโปรแกรมทั้ง 3 ครั้งของการได้รับยาเคมีบำบัด ผู้ป่วยมีคะแนนความสามารถในการดูแลช่องปากสูงกว่าก่อนได้รับโปรแกรมฯ อย่างมีนัยสำคัญทางสถิติ (p<.001) และความรุนแรงของภาวะเยื่อบุช่องปากอักเสบลดลงอย่างมีนัยสำคัญทางสถิติ (p<.01)

          การศึกษาครั้งนี้แสดงให้เห็นว่าโปรแกรมการดูแลช่องปากช่วยเพิ่มความสามารถในการดูแลช่องปากของผู้ป่วยมะเร็งลำไส้และมะเร็งเต้านมที่ได้รับเคมีบำบัด และมีประสิทธิภาพในการป้องกันและลดความรุนแรงของภาวะเยื่อบุช่องปากอักเสบจากการได้รับยาเคมีบำบัดได้

Effects of an Oral Care Program on Oral Care Ability and Oral Mucositis among Cancer Patients Receiving Chemotherapy

        This quasi-experimental research aimed to study the effects of an oral care program on the oral care ability and severity of oral mucositis among cancer patients receiving chemotherapy. Fifteen colorectal and breast cancer patients were purposively selected. An oral care program was developed by the researchers using an educative-supportive nursing system and the BC Cancer Agency’s oral care practice guideline. The program included teaching, guiding and directing, supporting, and providing appropriate environmental conditions. It was implemented during 3 consecutive cycles of chemotherapy. Data were collected before and after the intervention using oral care ability questionnaires, and an oral mucositis record form. Research instruments were validated by 3 experts. The CVI of oral care ability questionnaires was .90, and Cronbach’s alpha coefficient was .91. The data were analyzed by descriptive statistics, Friedman test, and Wilcoxon signed rank test. 

        Research results showed that after the intervention in all 3 cycles of chemotherapy, the patients had significantly higher oral care ability scores than before the intervention (p<.001) and the severity of mucositis was significantly reduced (p<0.01).

        The study revealed that the oral care program can enhance cancer patients’ oral care ability, and it is effective for preventing and reducing the severity of oral mucositis after chemotherapy in cancer patients.
