Abstract

ผู้ใช้บริการที่มีแผลกดทับต้องใช้เวลานานในการรักษาแผลให้หาย จึงจำเป็นที่จะต้องมีการวางแผนจำหน่ายอย่างมีประสิทธิภาพ เพื่อเตรียมผู้ใช้บริการและครอบครัวให้มีความพร้อมในการกลับไปดูแลต่อที่บ้าน การวางแผนจำหน่ายผู้ใช้บริการที่มีแผลกดทับเป็นบทบาทหน้าที่ของทีมสหสาขาวิชาที่รับผิดชอบดูแลผู้ใช้บริการร่วมกันเพื่อให้การดูแลแบบองค์รวม และจะต้องอาศัยความร่วมมือจากผู้ใช้บริการและครอบครัว พยาบาลเป็นหนึ่งในทีมสหสาขาวิชาที่เป็นผู้ที่ใกล้ชิดผู้ใช้บริการจึงจะต้องวางแผนจำหน่ายผู้ใช้บริการที่มีแผลกดทับโดยใช้กระบวนการพยาบาลในการดูแลผู้ใช้บริการ บทความนี้จะนำเสนอ ความสำคัญของการวางแผนจำหน่ายผู้ใช้บริการที่มีแผลกดทับ ปัจจัยส่งเสริมให้การวางแผนจำหน่ายมีประสิทธิภาพ บทบาทและหน้าที่ของพยาบาลในการวางแผนจำหน่ายผู้ใช้บริการที่มีแผลกดทับ และการวางแผนจำหน่ายผู้ใช้บริการที่มีแผลกดทับโดยใช้กระบวนการพยาบาล

Discharge planning in clients with pressure sore

The healing process of pressure sore would take long period of time so it is necessity to have continuing wound management with effective discharge planning. Healthcare providers need to prepare clients and caregivers in advance to be ready for self-care at home. Multidisciplinary team should not only be mingled in contribution of discharge planning that will lead to the provision of holistic care, but also it should have the contribution from clients and family in self-management. Nurse is one of healthcare team who could customize intervention upon individual pressure sore clients’ needs base on nursing process. In this article, there will present significance of discharge planning for clients with pressure sore, promoting factors for effective discharge planning, nurses’ role in discharge planning for pressure sore clients, and application of nursing process in discharge planning clients with pressure sore.
