ประสบการณ์การได้รับการสนับสนุนทางสังคมในผู้ป่วยไตวายเรื้อรังระยะสุดท้ายที่ได้รับการฟอกเลือดด้วยเครื่องไตเทียม

Abstract

     การวิจัยเชิงคุณภาพนี้มีวัตถุประสงค์เพื่อศึกษาประสบการณ์การได้รับการสนับสนุนทางสังคมในผู้ป่วย ไตวายระยะสุดท้ายที่ได้รับการฟอกเลือดด้วยเครื่องไตเทียม  การวิจัยใช้ระเบียบวิธีวิจัยแบบปรากฏการณ์วิทยา ตามแนวคิดของ Edmund  Husserl ผู้ให้ข้อมูลทั้งหมด 18 คนแบ่งเป็นผู้ป่วยที่รักษาด้วยการฟอกเลือดด้วยเครื่องไตเทียม จำนวน 12 คนและญาติที่ดูแล 6 คน เก็บรวบรวมข้อมูลโดยการสัมภาษณ์แบบเจาะลึก  ตั้งแต่ 1 สิงหาคม ถึง 31 ตุลาคม 2557 วิเคราะห์ข้อมูลโดยการวิเคราะห์เนื้อหา

     ผลการศึกษา พบ 3 ประเด็นหลัก ดังนี้  1. ด้านปัญหา คือ ค่าใช้จ่ายสูงและต้องจ่ายอย่างต่อเนื่อง ขาดบุคคลที่ให้คำชี้แนะการใช้สิทธิการรักษา ขาดผู้ดูแล ผู้ป่วยไม่ควบคุมการรับประทานอาหาร ความทนในการทำกิจกรรมลดลง  2. ด้านความต้องการ คือ การช่วยเหลือค่าใช้จ่าย การแนะนำวิธีการใช้สิทธิการรักษา การให้ความรู้ทางสุขภาพ 3. ด้านการได้รับการดูแลด้านอาหารและยา   

     ข้อเสนอแนะ หน่วยบริการการฟอกเลือดด้วยเครื่องไตเทียมควรจัดบริการให้การปรึกษาที่เป็นรูปแบบเกี่ยวกับการเข้าถึงสิทธิการรักษาของผู้ป่วย การให้ข้อมูลอย่างต่อเนื่องเพื่อเพิ่มความรู้แก่ผู้ป่วยและผู้ดูแล

 Experience of Using Social Support in End Stage Renal Disease Patients Receiving Hemodialysis

     This qualitative research aimed to explore experience of social support in end stage renal disease patients receiving hemodialysis. Phenomenological method of Edmund  Husseri was applied as the research methodology. The eighteen informants comprised twelve patients and six caregivers. Data were collected through in-depth interviews, since August 1 to October 31, 2014. Content analysis was used in this study.

     The findings consisted of 3 major factors as follows: 1) Problems-highly cost for hemodialysis and required a continuing healthcare costs, lack of individual guidance for health insurance rights, lack of  caregiver, uncontrol diet, and decreased endurance in activity 2) Demands-supporting healthcare cost, advising health insurance rights, and providing comprehensive health education 3) Nutrition and medication management.

     Research suggestions are that the hemodialysis unit should provide counseling about health insurance rights for end stage renal disease patients, and continuingly support in formation for patients and caregivers.
