Incidences and Causes of Reintubation after Undergoing General Anesthesia at Songkhla Hospital

The aim of this retrospective study was to analyze the incidences and the risk factors correlated to reintubation after general anesthesia. The study was conducted between October 1, 2016, and September 30, 2019. The data, including sex, age, ASA Physical status (PS), time of incidence, type of operation, department of operation, place of incidence, duration of operation, and time of reintubation, were collected from 23,071 patients in anesthetic databases, anesthetic statistics, medical records and risk data in anesthesiology departments. Factors related to reintubation were analyzed by the researcher, a nurse anesthetist in risk management team, and anesthetists. Data were analyzed by using descriptive statistics.
Descriptive results showed an incidence of reintubation after general anesthesia of 20 cases in total. The percentage of reintubation was 0.086 or 8.6 per 10,000 patients who received general anesthesia with endotracheal tube. The main causes (55%) of factors that led to reintubation of patients were: 1) upper airway obstruction, 2) chronic obstructive pulmonary disease, 3) pulmonary edema, 4) sepsis, 5) seizures, 6) laryngospasm and acute myocardial infarction. The two main causes of factors associated with anesthesia that led to reintubation (35%) were residual effect of anesthetic agent, and respiratory depression from narcotics. Factors associated with surgery that led to reintubation (10%) were recurrent laryngeal nerve injury and surgical bleeding.
In conclusion, causes of reintubation after general anesthesia included three factors. The main causes were underlying diseases, anesthesia, and surgery. Those factors related to reintubation must be taken into account, so that reintubation incidence will be decreasing.
