The Effects of Pharmaceutical Care on Chronic Kidney Disease Patients in Stage 1- 4 at Phrom Khiri Hospital
Abstract
Pharmaceutical care is the direct, responsible provision of medication-related care for the purpose of achieving definite outcomes that improve a patient’s quality of life. This quasi-experimental research, one-group pre test-post test design, aimed to: 1) study knowledge about drug use, chronic kidney disease as well as medication adherence, both before and after pharmaceutical care, and 2) evaluate the effectiveness of pharmaceutical care to finding and solving problems from drug use in patients with stage 1-4 of chronic kidney disease who received treatment at Phrom Khiri Hospital of Nakon Si Thammarat. Population was 162 diabetes patients and/or hypertension patients with chronic kidney disease at stages 1-4 who received treatment at the hospital and in 2022. The sample was 60 patients selected by simple random sampling. The experimental tool was the pharmaceutical care protocol. The experiment was implemented for 1-2 month. Data were collected using questionnaire between July 1st to December 31st 2022, and were analyzed using percentage, mean, standard deviation and paired t-test.
The results showed that before the experiment, most of patients (76.66%) had knowledge about drug use at a low level. However, after the experiment, almost all patients (98.33%) had knowledge about drug use at a high level. In terms of knowledge about chronic kidney disease, the knowledge level before and after the experiment were at a low level (68.33%), and at a high level (78.33%), respectively. Medication adherence before the experiment were poor at 83.33% and after the experiment were good at 71.67%. When compare between before and after the experiment knowledge about drug use, knowledge about chronic kidney disease and medication adherence were significantly increased. As for outcome of pharmaceutical care, it was found that after operation, prescription of ACEIs/ARBs had a significantly increased proportion (p<0.05).
Per findings, there should be a service to provide more suitable computer information system to record and analyze patients' data.
