Risk Factors for Postoperative Pneumonia in Elderly Patients with hip fractures at Phukhieo Chaleom Phra Keit Hospital Chaiyaphum Province.

Elderly patients with hip fractures found pneumonia 5.1-14%. Mortality rate in the first 30 day after surgery 27-43%. Readmission increased eight times compared to patients without pneumonia. Retrospective descriptive study of medical records. Elderly patients with hip fractures that age more than or equal to 50 years from Fragility fracture. There were 227 cases, 18 case of pneumonia accounted for 7.93% of the total population, 209 patients without pneumonia accounted for 92.07% of the total population. Risk factors studies include age, gender, BMI, smoking, amount of time and duration from the first admission to the hospital until the time of surgery, serum albumin level, blood sugar level, ASA Classication, use of psychotropic, underlying diseases, length of hospital stay and physical therapy programs. The data were statistically significant differences when P valve < 0.05. The results showed that the risk factor for pneumonia is chronic obstructive pulmonary disease(P valve=0.035) and ASA classification(P valve=0.028). Pneumonia were found it at least in patients receiving physical therapy on the first day admitted to the hospital(P valve=0.005). Pneumonia causes increased length of hospital stay(P valve=0.003).Concluded that physical therapy on the first day admitted to the hospital to prevent pneumonia. Patients with chronic obstructive pulmonary disease, ASA classification >3 and patients had been hospitalized for a long time in groups be high risk of pneumonia. They should be taken to monitor pneumonia. All patients should get physical therapy since the first day admitted to the hospital.
