The Success of Patients Dying at Their Preferred Place of Death as Specified in Advance Care Planning among Terminal Cancer Patients At Saraburi Hospital

Abstract

Objectives: To evaluate the success rate of terminal cancer patients dying at their preferred place of death as specified in advance care planning (ACP) under the palliative care team at Saraburi hospital; and to analyze factors influencing the success of dying at home as per patients’ wishes.

Methods: Prospective cross-sectional study included 53 terminal cancer patients who had completed ACP and were under the care of the palliative team at Saraburi Hospital between July 1, 2024, and November 30, 2024. Data collected included patient demographics, clinical characteristics, and preferred place of death. After death, the actual place of death was recorded, and factors influencing the success of dying at home were analyzed.

Results: Among 53 patients, 36 patients (69.8%) died at their preferred location. Patients residing in Mueang district and those cared for by their children were more likely to die at home, but this was not statistically significant (OR=1.19, 95% CI: 0.27-5.25 and OR=2.08, 95% CI: 0.5-8.71, respectively). Patients experiencing pain or dyspnea were less likely to die at home, but the association was not statistically significant (OR=0.5, 95% CI: 0.06-5.35 and OR=0.9, 95% CI: 0.24-3.45, respectively).

Conclusion: The majority of terminal cancer patients receiving palliative care died at their preferred location. However, no specific factors were found to significantly influence the success of dying at home.
