การทำฟันเทียมทั้งปากในผู้ป่วยที่มีสันเหงือกล่างละลายรุนแรง โดยใช้เทคนิคเขตเป็นกลาง

ผู้ไร้ฟันที่มีสันเหงือกล่างละลายรุนแรงมักมีปัญหาเสถียรภาพและการยึดอยู่ของฟันเทียม เทคนิคเขตเป็นกลาง (neutral zone) ที่จัดตำแหน่งฟันและพื้นผิวขัดมันให้สมดุลกับแรงลิ้น-แก้ม-ริมฝีปาก ช่วยเพิ่มผลลัพธ์ที่ผู้ป่วยรายงานได้ดีกว่าวิธีดั้งเดิม
วัตถุประสงค์ เพื่อนำเสนอการทำฟันเทียมทั้งปากด้วยเทคนิค Neutral zone ในผู้ป่วยที่สันเหงือกละลายอย่างรุนแรง ที่สามารถทำได้ในในโรงพยาบาลชุมชน
วิธีการ ขากรรไกรบนทำตามแนวทางมาตรฐาน ส่วนขากรรไกรล่างบันทึก neutral zone ด้วย admixed compound ทำดัชนีซิลิโคนถ่ายทอดรูปทรง ใช้การสบแบบ monoplane มีเรียงฟันหลังและฟันหน้ามีการสบคร่อมฟันเล็กน้อยให้สอดรับความกว้างลิ้น และขึ้นรูปพื้นผิวขัดมันเชิงหน้าที่
ผลลัพธ์ การยึดอยู่ ความเสถียรภาพเป็นที่น่าพอใจ เคี้ยวสบาย พูดชัดเจน พบจุดกดเจ็บเล็กน้อยที่ 3 วันและแก้ไขได้ และจากการติดตามในเดือนที่ 1, 3, 6 เดือน ไม่พบแผลกดซ้ำ
สรุป เทคนิคนี้ทำได้ง่ายเหมาะในรายสันเหงือกล่างละลายรุนแรง เมื่อไม่อาจทำรากเทียม ช่วยเพิ่มเสถียรภาพและความพึงพอใจ
