Abstract

Hidradenitis suppurativa (HS) or acne inversa is a rare disease. The pathogenesis is follicular occlusion that results in the inflammation of the apocrine gland. The usual clinical presentation includes chronic recurrent painful erythematous papules and pustules at both axillae, both sides of the groin, the occipital hairline and intermammary areas. These pustules spontaneously rupture and form sinus drainages and scars.

In this case, a 19 year-old Thai female from Petchaburi Province, Thailand had a clinical presentation that was compatible with severe HS: multiple painful papules and pustules with sinus tract and scars occurring in the common sites of HS. Despite the severity of her case, most of her lesions improved after she took rifampicin and clindamycin for ten weeks.
