Treatment of Hypertrophic scars and Keloids with Self-adhesive Polyurethane Dressing (Hansaplast scar reducer) in Thai Patients

Hypertrophic scars and keloids are common in Thai patients. Many treatment modalities are available but not uniformly successful. This study is a prospective single-blinded intra- individual comparative study to evaluate the efficacy and side effect of a self –adhesive polyurethane dressing (Hansaplast scar reducer) compared to a fabric adhesive dressing (Fixomull) for treatment of hypertrophic scars and keloids in 36 Thai patients. The volunteers applied the 2 dressings on the scars 12 hours per day for 8 weeks. Evaluation of scar and side effects was done by doctor and patients during follow up. Thirty patients completed the 8 week treatment period. Two patients withdrawn due to severe dermatitis on both treatment areas. Both the polyurethane dressing and fabric dressing significantly improved the scars. The patients were more satisfied with the improvement from the polyurethane dressing than the fabric dressing. The positive effects were evident after 4 weeks of application .The Vancouver scar scale, clinical and photograph evaluation by doctor found that both dressings significantly improved the scars but the efficacy of the 2 dressings were not significantly different. The side effects of the 2 dressings were not different and were mainly itch.
In summary, treatment of hypertrophic scars and keloids in Thai patients with the commercial self-adhesive polyurethane dressing significantly improved the scar symptoms and appearance with high degree of satisfaction and minimal side effects.
