Methylene Blue for Vasoplegic Syndrome after Cardiac Surgery

Vasoplegic syndrome (VPS) is a form of distributive shock commonly observed after cardiac surgery. It is characterized by profound hypotension and markedly reduced systemic vascular resistance despite normal or elevated cardiac output. The pathophysiology involves activation of the nitric oxide pathway and a reduction in vasopressin levels, leading to vascular smooth muscle relaxation and diminished responsiveness to conventional vasopressors. VPS occurs in approximately 5%–25% of postoperative cardiac surgical patients and is associated with mortality rates of up to 30%–50% in patients who remain refractory to vasopressors for more than 36 hours. Current therapeutic strategies recommend a stepwise vasopressor approach, with norepinephrine as the first-line agent, followed by vasopressin or terlipressin as second-line agents, and methylene blue as the third-line therapy. Methylene blue exerts its effects by inhibiting nitric oxide synthase and guanylyl cyclase, thereby reducing cyclic guanosine monophosphate formation and restoring vascular smooth muscle contractility. The recommended dosing regimen of methylene blue is an intravenous bolus of 1–2 mg/kg administered over 15–30 minutes, followed by continuous infusion at 0.5–1 mg/kg/hr. Evidence from clinical studies indicates that methylene blue may enhance mean arterial pressure, reduce vasopressor requirements, and potentially decrease mortality in selected patient populations. Common adverse effects include bluish-green urine discoloration, and caution is advised regarding serotonin syndrome when used concomitantly with serotonergic medications. Available evidence supports methylene blue as a beneficial adjunct in the management of VPS following cardiac surgery, particularly in cases refractory to standard vasopressor therapy. However, further high-quality studies are required to define optimal dosing strategies, timing of administration, and long-term safety profiles.
