Insulin Icodec, a new type of insulin for treating diabetes.
Keywords:

Diabetes is a chronic non-communicable disease that has been continuously increasing in Thailand, especially type 2 diabetes, in which insulin resistance is a key factor in disease development. Treatment of type 2 diabetes includes lifestyle modification and blood sugar-lowering medications, including once-daily basal insulin injections that have limitations regarding injection frequency and risk of hypoglycemia.
Insulin Icodec is an ultra-long-acting insulin analog for once-weekly injection developed to improve convenience and adherence in the treatment of type 1 and type 2 diabetes. With a molecular structure optimized for high stability and strong binding to plasma albumin, it has a half-life of approximately 196 hours. The ONWARDS Trials (1-6) showed that insulin Icodec is as effective or superior in controlling glycated hemoglobin (HbA1c) compared to once-daily basal insulins such as insulin glargine and insulin degludec. It is also safe with an acceptable rate of hypoglycemia, especially in patients with type 2 diabetes, both insulin-injecting and insulin-injection naïve patients. The most common adverse effects are hypoglycemia and injection site reactions.
With a molecular structure allowing continuous action for over one week and a concentration of 700 units/mL, insulin Icodec is a new option that reduces injection burden and increases convenience in managing diabetes in adults, reduces treatment barriers, and has the potential to close gaps in diabetes care systems in Thailand.
