Sodium Zirconium Cyclosilicate: A Novel Agent for the Management of Hyperkalemia in Adult Patients

Hyperkalemia is a potentially life-threatening condition commonly observed in patients with diabetes mellitus, cardiovascular disease, and chronic kidney disease (CKD), particularly those receiving renin-angiotensin-aldosterone system inhibitors (RAASi). Although RAASi therapy offers well-established renal and cardiovascular protective benefits, drug-induced hyperkalemia frequently necessitates dose reduction or discontinuation, limiting the optimal use of these essential therapies. In recent years, potassium-binding agents have been developed to solve this clinical challenge. These include calcium polystyrene sulfonate, patiromer, and sodium zirconium cyclosilicate (SZC). SZC was recently approved by the Thai Food and Drug Administration in July 2024 for the treatment of hyperkalemia in adult patients. SZC is a non-polymeric, non-absorbed inorganic crystalline compound that selectively binds potassium ions in the gastrointestinal tract. It has demonstrated rapid, sustained potassium-lowering effects and high specificity for potassium, with clinical data supporting its efficacy and safety.
