ปัจจัยทำนายคุณภาพชีวิตผู้ป่วยมะเร็งตับที่รับการรักษาด้วยการให้ยาเคมีบำบัดทางสายสวนหลอดเลือดแดง

วัตถุประสงค์การวิจัย: เพื่อศึกษาความสัมพันธ์เชิงทำนายระหว่างปัจจัยด้านอาการอ่อนล้านอนไม่หลับและเบื่ออาหารต่อคุณภาพชีวิตผู้ป่วยมะเร็งตับที่รับการรักษาด้วยการให้ยาเคมีบำบัดทางสายสวนหลอดเลือดแดงในระยะ 48- สัปดาห์
การออกแบบการวิจัย: การวิจัยความสัมพันธ์เชิงทำนาย
การดำเนินการวิจัย: กลุ่มตัวอย่างเป็นผู้ป่วยที่ได้รับการวินิจฉัยว่าเป็นมะเร็งตับภายหลังได้รับการรักษาด้วยยาเคมีบำบัดทางสายสวนหลอดเลือดแดงและมาตรวจติดตามอาการในระยะ48- สัปดาห์ ภายหลังการรักษา ณ หน่วยตรวจผู้ป่วยนอก จำนวน77คน ด้วยชุดแบบสอบถาม ประกอบด้วย แบบบันทึกข้อมูลส่วนบุคคล แบบประเมินอาการอ่อนล้า อาการนอนไม่หลับ อาการเบื่ออาหารและคุณภาพชีวิตผู้ป่วยมะเร็งตับ ค่าความเชื่อมั่นของเครื่องมือ เป็น 0.97, 0.87, 0.84 และ 0.71 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนาและการวิเคราะห์สมการถดถอยพหุคูณแบบขั้นตอน
ผลการวิจัย: กลุ่มตัวอย่างมีอาการอ่อนล้าเล็กน้อย (X =31.19, SD =27.09) ไม่มีอาการนอนไม่หลับ(X= 6.39, SD= 7.74)ขณะที่มีอาการเบื่ออาหารระดับปานกลาง(X= 17.25, SD=5.55)และมีคุณภาพชีวิตในระดับดี ( X = 138.36, SD = 22.99) อาการอ่อนล้า พบว่า มีอิทธิพลต่อคุณภาพชีวิตมากที่สุด (ß=-.49, p<.001) รองลงมาคือ อาการเบื่ออาหาร (ß=-.38, p<.001) และอาการนอนไม่หลับ(ß=-.18, p<.05) โดยสามารถร่วมกันทำนายการเปลี่ยนแปลงคุณภาพชีวิตได้ร้อยละ 80 (R2= .80,F=.96, p<.001)
ข้อเสนอแนะ: พยาบาลควรประเมินความรุนแรงของอาการอ่อนล้า เบื่ออาหาร และนอนไม่หลับในผู้ป่วยมะเร็งตับหลังรับการรักษาด้วยยาเคมีบำบัดทางสายสวนหลอดเลือดแดงเพื่อวางแผน ควบคุมหรือบรรเทาอาการก่อนที่อาการจะรุนแรงและส่งผลต่อคุณภาพชีวิตของผู้ป่วย
