การสื่อสารข้ามวัฒนธรรมทางการพยาบาล: ทักษะที่จำเป็นในศตวรรษที่ 21

ในยุคศตวรรษที่ 21 ที่มีการเปลี่ยนแปลงของสุขภาพโลก ทั้งในด้านโครงสร้างประชากร ความชับช้อนของปัญหาทางสังคมและความหลากหลายทางวัฒนธรรมที่มีผลต่อระบบบริการสุขภาพ การสื่อสารข้ามวัฒนธรรม เป็นทักษะหนึ่งที่มีความสำคัญในการประเมิน ค้นหาปัญหา และวางแผน การพยาบาล เน้นการตอบสนองความต้องการของผู้ใช้บริการเป็นสำคัญ ดังนั้น พยาบาบาลเป็นหนึ่งในผู้ให้บริการที่ต้องมีความเข้าใจเกี่ยวกับการสื่อสารข้ามวัฒนธรรม บทความนี้มีวัตถุประสงค์เพื่อ 1) ทบทวนปัจจัยสำคัญในการสื่อสารข้ามวัฒนธรรม การประเมินวัฒนธรรมการสื่อสารของผู้ส่งสาร และผู้รับสาร ทักษะในการสื่อสารข้ามวัฒนธรรมทางการพยาบาลและ 2) นำเสนอแนวปฏิบัติในการสื่อสาร ข้ามวัฒนธรรมทางการพยาบาล ปัจจัยสำคัญในการสื่อสารข้ามวัฒนธรรม ประกอบด้วย 1) ปัจจัยด้านผู้รับสาร ได้แก่ ความพร้อมด้านร่างกาย ทักษะด้านภาษา ทัศนคติ ความรู้และประสบการณ์เกี่ยวกับ ประเด็นที่สื่อสาร และทักษะในการฟัง 2) ปัจจัยด้านผู้ส่งสาร ได้แก่ ความสามารถทางภาษา ความเข้าใจ ในวัฒนธรรม ทัศนคติที่ดีต่อการดูแลผู้ที่มีความต่างทางวัฒนธรรม และสมรรถนะทางวัฒนธรรมหรือความไวทางวัฒนธรรม และ 3) ปัจจัยด้านสิ่งแวดล้อม ได้แก่ สภาพแวดล้อมของการสื่อสาร บรรยากาศ ในการสนทนา พลังในการสร้างบรรยากาศที่ดี และการสัมผัส พยาบาลควรมีความสามารถดังนี้ 1) การประเมินวัฒนธรรมการสื่อสารของผู้ส่งและผู้รับสาร ประกอบด้วย ความเชื่ออำนาจในด้านสุขภาพ ความเชื่อในการแสดงออกทางอารมณ์ พฤติกรรมการแสวงหาข้อมูล ความเชื่อเกี่ยวกับการประสานสายตา และการให้ความหมายด้านเวลา และ 2) ทักษะในการสื่อสารข้ามวัฒนธรรมทางการพยาบาล เพื่อให้เกิดความเข้าใจในการดูแลผู้ที่มีความต่างวัฒนธรรมประกอบด้วย ทักษะการฟังอย่างตั้งใจ ทักษะการตรวจสอบ ความเข้าใจ ทักษะการประเมินย้อนกลับ ทักษะการสนทนาแบบมีเป้าหมาย ทักษะการเป็นกระจกสะท้อน ทักษะการสร้างสัมพันธภาพ ทักษะการเปิดเผยตนเอง และทักษะการสร้างสัมพันธภาพเพื่อการบำบัด แนวปฏิบัติในการสื่อสารข้ามวัฒนธรรมทางการพยาบาล ได้แก่ 1) การประเมินความเชื่อต่อบุคคลที่มีความต่างทางวัฒนธรรม 2) การประเมินปัจจัยที่อาจมีผลหรือเป็นอุปสรรคต่อการสื่อสารระหว่างพยาบาลและผู้ป่วย 3) การวางแผนและปฏิบัติการพยาบาล 4) การปรับวิธีการสื่อสารให้สอดคล้องกับความต้องการ และพื้นฐานทางวัฒนธรรม 5) การสื่อสาร 6) วิธีการสื่อสารในการให้ข้อมูลหรือสอน 7) ท่าทีที่ใช้ในการสื่อสาร 8) การใช้เทคนิคการสื่อสารที่เหมาะสม 9) การใสใจและติดตามผลเป็นระยะ และ 10) การใช้ล่ามเพื่อเสริมความเข้าใจในเนื้อหาเพื่อการสื่อสาร นอกจากนี้ บางกรณีอาจจำเป็นต้องใช้ล่ามเพื่อเสริมความเข้าใจในเนื้อหามากขึ้น การพัฒนาพยาบาลให้มีความสามารถเป็นล่ามได้ด้วยตนเอง จะส่งผลดีต่อคุณภาพการดูแลเพราะพยาบาลมีความเข้าใจการเจ็บป่วย ทั้งนี้การทำงานร่วมกับล่าม ล่ามต้องมีมาตรฐานจริยธรรมของล่าม และกำหนดแนวทางการทำงานร่วมกับล่ามให้ชัดเจน การพัฒนาพยาบาลให้มีความสามารถการสื่อสารข้ามวัฒนธรรม เป็นการพัฒนาคุณภาพการพยาบาลในศตวรรษที่ 21 ที่สำคัญอย่างยิ่ง ข้อเสนอแนะมีดังนี้ 1) หน่วยงานควรจัดให้มีการอบรม (Inservice education) เพื่อเตรียมความพร้อมให้กับพยาบาล ทั้งด้านทักษะทางภาษา ความเข้าใจทางวัฒนธรรรม และการฝึกฝน ทักษะการแก้ไขปัญหาที่เกิดจากความแตกต่างทางวัฒนธรรม ซึ่งมีแนวโน้มที่จะเพิ่มมากขึ้นอย่างต่อเนื่อง 2) การใช้แอปพลิเคชันช่วยในการสื่อสารและต้องคำนึงถึงข้อจำกัดของสื่อนั้น ๆ เพื่อป้องกันความผิดพลาดในการดูแลผู้ป่วย
