บทคัดย่อ

บทนำ ภาวะโรคเรื้อรังร่วมหลายโรคพบมากในผู้สูงอายุและมีความสัมพันธ์กับอัตราตายที่สูงขึ้น เมื่อเข้าสู่วาระสุดท้ายผู้สูงอายุที่ไม่ได้มีการเตรียมความพร้อมจะส่งผลให้คุณภาพการตายไม่ดี โดยความรอบรู้เกี่ยวกับความตายเป็นความรู้และทักษะที่จำเป็นสำหรับบุคคลเพื่อการเข้าถึง เข้าใจ และตัดสินใจเลือกทางเลือกในวาระสุดท้ายของชีวิตและความตาย โดยปัจจัยที่เกี่ยวข้องกับความรอบรู้ ด้านสุขภาพประกอบด้วย 6 ปัจจัย ได้แก่ ทัศนคติต่อความตาย ทัศนคติต่อการดูแลระยะสุดท้าย ความวิตกกังวลเกี่ยวกับความตาย ความเชื่อด้านศาสนา การปฏิบัติกิจกรรมทางศาสนา และการสนับสนุนทางสังคม 

วัตถุประสงค์การวิจัย เพื่อศึกษาความสัมพันธ์ระหว่างทัศนคติต่อความตาย ทัศนคติต่อการดูแลระยะสุดท้าย ความวิตกกังวลเกี่ยวกับความตาย ความเชื่อด้านศาสนา การปฏิบัติกิจกรรมทางศาสนา และการสนับสนุนทางสังคมกับความรอบรู้เกี่ยวกับความตายของผู้สูงอายุที่มีโรคเรื้อรังร่วมหลายโรค 

การออกแบบการวิจัย การศึกษานี้เป็นแบบพรรณนาเชิงความสัมพันธ์ ใช้แนวคิดความรอบรู้ เกี่ยวกับความตายซึ่งต่อยอดมาจากแนวคิดความรอบรู้ด้านสุขภาพซึ่งโซเรนเซนและคณะ ที่อธิบายว่าเป็นความรู้ สมรรถนะหรือความสามารถ และแรงจูงใจ และได้กำหนดปัจจัยที่เกี่ยวข้องกับความรอบรู้ ด้านสุขภาพ 3 ปัจจัยหลัก ได้แก่ ปัจจัยด้านบุคคล ปัจจัยด้านสถานการณ์ และปัจจัยด้านสังคมและสิ่งแวดล้อม โดยการวิจัยครั้งนี้เลือกปัจจัยที่คาดว่าจะมีความสัมพันธ์กับความรอบรู้เกี่ยวกับความตาย ตามรูปแบบของโซเรนเซนและคณะ คือ ความวิตกกังวลเกี่ยวกับความตาย ทัศนคติต่อความตาย ทัศนคติต่อการดูแลระยะสุดท้าย ความเชื่อด้านศาสนาการปฏิบัติกิจกรรมทางศาสนา และการสนับสนุนทางสังคม 

การดำเนินการวิจัย กลุ่มตัวอย่างเป็นผู้ที่อายุ 60 ปีขึ้นไปที่ได้รับการวินิจฉัยด้วยโรคเรื้อรัง ตั้งแต่ 2 โรคขึ้นไป จำนวน 240 คน ที่มีการรู้คิดปกติ นับถือศาสนาพุทธ สื่อสารภาษาไทยและยินยอมเข้าร่วมการวิจัย คัดเลือกกลุ่มตัวอย่างแบบขั้นตอนคือคัดเลือกอำเภอเมืองจังหวัดเชียงใหม่และสุ่ม ตำบลตามภูมิศาสตร์ อย่างละ 1 ตำบล และสุ่มเลือกกลุ่มตัวอย่างที่ลงชื่อสมัครใจเข้าร่วมการวิจัยตามสัดส่วนประชากรแต่ละตำบล เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูลได้แก่ แบบวัดความรอบรู้เกี่ยวกับความตาย แบบวัดทัศนคติต่อความตาย แบบวัดทัศนคติต่อการดูแลระยะสุดท้าย แบบสอบถามความเชื่อทางศาสนา แบบสอบถามการปฏิบัติกิจกรรมทางศาสนาและแบบวัดการสนับสนุนทางสังคม เครื่องมือทุกส่วนได้ผ่านการตรวจสอบคุณภาพด้านความตรงตามเนื้อหาสำหรับแบบวัดฉบับแปลเป็นภาษาไทย ได้แก่ แบบวัดทัศนคติต่อความตาย และแบบวัดความรอบรู้เกี่ยวกับความตาย โดยมีค่าดัชนีความตรงตามเนื้อหามากกว่า .80 สำหรับความเชื่อมั่น ทุกแบบวัดมีค่าสัมประสิทธิ์ แอลฟาครอนบาค มากกว่า .80 วิเคราะห์ข้อมูลโดยใช้สถิติบรรยายและสหสัมพันธ์ของเพียร์สัน

ผลการวิจัย ความรอบรู้เกี่ยวกับความตายมีคะแนนเฉลี่ย 7.77 (SD =1.05) จากคะแนนเต็ม 10 คะแนน ทัศนคติต่อความตาย ความวิตกกังวลเกี่ยวกับความตาย ความเชื่อทางศาสนา การปฏิบัติกิจกรรมทางศาสนาและการสนับสนุนทางสังคมอยู่ในระดับปานกลาง ทัศนคติต่อการดูแลระยะสุดท้าย เชิงบวก ทัศนคติต่อการดูแลระยะสุดท้าย ทัศนคติต่อความตายและการสนับสนุนทางสังคมมีความสัมพันธ์ทางบวกในระดับต่ำกับความรอบรู้เกี่ยวกับความตายที่ p<.05 (r=.126, .1333, และ .133 ตามลำดับ) และการปฏิบัฏิบัติกิจกรรมทางศาสนาที่ p<.01 (r=.193) แต่ความวิตกกังวลเกี่ยวกับความตายและความเชื่อทางศาสนามีความสัมพันธ์ทางลบในระดับต่ำกับความรอบรู้เกี่ยวกับความตายที่ p<.01 (r= -.181, -.194 ตามลำดับ)

ข้อเสนอแนะ ผลการศึกษาเป็นแนวทางให้กับผู้ที่เกี่ยวข้องในการดูแลผู้สูงอายุในการส่งเสริมความรอบรู้เกี่ยวกับความตายโดยปรับทัศนคติต่อความตายและทัศนดติต่อการดูแลระยะสุดท้าย ลดความวิตกกังวลเกี่ยวกับความตายและส่งเสริมการสนับสนุนทางสังคมและการปฏิบัติกิจกรรมทางศาสนา
