Outcomes of Expanded Chronic Care Model Utilization in Patients with Chronic Obstructive Pulmonary Disease

Abstract
Tha Wang Pha hospital, Nan province, employed lung rehabilitation program for use with COPD patients under the collaboration of health professionals from the hospital and health promoting hospitals. Barriers existed when patients were relying on health system. Then, the hospital utilized Expanded Chronic Care Model, ECCM, in three phases: 1) Preparation phase, analyzed the readiness and barriers of the hospital and HPHs, prepared proactive community partner and practice team; 2) Implementation phase, based on each components of the model were, manage health system organization, clinical information system, decision making support, and delivery system design/re-orient health services. Implementations in the community were, built healthy public policy, developed self-management/personal skills, and strengthened community action; 3) Evaluation phase, clinical outcomes, patients impact and patients’ satisfaction were better for COPD patients under the model. It is suggested that this model can be use for chronically ill patients for better outcomes.
