Development of Mobile Application to Promote Continuity of Care from Hospital to Community for Patients with Severe Multiple Trauma

Abstract
This research and development aimed to develop a mobile application to promote continuity of care for patients with severe multiple trauma from hospital to community in one area of Mueang District Songkhla province. The study focused on development of the mobile application phase and evaluation of user’s satisfaction and feasibility. The participants were purposively selected, and consisted of eight nurses working at Songkhla hospital and community health center, 27 village health volunteers, and five caregivers of patients with severe multiple trauma. The instruments used were semi-structured interview guides and the assessment of user satisfaction in using the Continuity of Care Model (CCM) application. The data were analyzed using content analysis and descriptive statistics. Results showed that: 1) mobile application for promoting continuity of care for patients with severe multiple trauma from hospital to community consisted of three parts regarding user group, namely caregivers, village health volunteers, and administrators, and 2) the mobile application was feasible to be applied, reflected by a high level of satisfaction of caregivers, village health volunteers, and application administrators. In addition, the caregivers reported their satisfaction in using the application and the possibility to be used for improving accessibility and communication with healthcare providers, and as a learning resource for patient care. However, larger samples and testing the application for its effectiveness are necessary.
