นวัตกรรมระบบการดูแลผู้สูงอายุภาวะสมองเสื่อมครบวงจร: โมเดลอำเภอบ้านฝาง

บทคัดย่อ
ระบบการดูแลผู้สูงอายุภาวะสมองเสื่อมโดยเฉพาะการวินิจฉัยรักษาและดูแล เป็นบริการที่ยังจำกัดในระดับตติยภูมิเท่านั้น แม้จะมีการคัดกรองภาวะสมองเสื่อมในระดับปฐมภูมิแต่ยังขาดการวินิจฉัยรักษาดูแลที่ต่อเนื่องและครบวงจร บทความนี้ นำเสนอระบบการดูแลผู้สูงอายุภาวะสมองเสื่อมครบวงจร: โมเดลอำเภอบ้านฝาง ซึ่งเป็นนวัตกรรมระบบบริการสุขภาพที่ได้มีการพัฒนาขึ้นจากงานวิจัยเชิงปฏิบัติการ เรื่อง การพัฒนาระบบการดูแลผู้สูงอายุภาวะสมองเสื่อมครบวงจร: กรณีศึกษาอำเภอบ้านฝาง จังหวัดขอนแก่น ที่มีระบบย่อย 3 ระบบ คือ 1) ระบบคัดกรองและคัดแยก: แบบลงแขก ลงขัน 2) ระบบการวินิจฉัยบำบัดรักษา: สภาลูกขุน และ 3) ระบบการดูแลต่อเนื่องในชุมชน: ไตรภาคี ซึ่งทั้งสามระบบย่อยนี้ มีการดูแลที่เชื่อมโยงกันทั้งระดับปฐมภูมิ ทุติยภูมิ และตติยภูมิ โดยเฉพาะ รูปแบบของ “สภาลูกขุน” ในโรงพยาบาลชุมชน ถือเป็นจุดเชื่อมต่อสำคัญ ทำให้ผู้สูงอายุที่มีภาวะสมองเสื่อมและครอบครัวเข้าถึงบริการการวินิจฉัยบำบัด รักษาและการดูแลอย่างต่อเนื่องจนครบวงจร โดยมีข้อเสนอแนะในการพัฒนาระบบอย่างต่อเนื่องและขยายผลต่อไป
