การพัฒนารูปแบบการดูแลแบบบูรณาการแก่ผู้ป่วยบาดเจ็บศีรษะโดยการนำของพยาบาล

บทคัดย่อ
การวิจัยและพัฒนานี้ มีวัตถุประสงค์เพื่อ 1) พัฒนารูปแบบการดูแลแบบบูรณาการแก่ผู้ป่วยบาดเจ็บศีรษะโดยการนำของพยาบาล ในการป้องกันภาวะแทรกซ้อน คือ การติดเชื้อทางเดินปัสสาวะจากการคาสายสวนปัสสาวะ การเกิดภาวะปอดอักเสบจากการใช้เครื่องช่วยหายใจ และการจัดการด้านโภชนาการ และ 2) ทดลองใช้รูปแบบที่พัฒนาขึ้น ที่หอผู้ป่วยศัลยกรรมประสาท โรงพยาบาลวชิระภูเก็ต ระหว่างเดือนมีนาคม–พฤศจิกายน พ.ศ. 2562 ผลการพัฒนา ได้รูปแบบการดูแลแบบบูรณาการโดยการนำของพยาบาลสำหรับผู้ป่วยบาดเจ็บศีรษะ มี 3 องค์ประกอบ คือ 1) นโยบายการบูรณาการงานวิจัยในการดูแลผู้ป่วยบาดเจ็บศีรษะ 2) แนวปฏิบัติการดูแลผู้ป่วยบาดเจ็บศีรษะ และ 3) การพัฒนาสมรรถนะพยาบาล ผลการศึกษา ผลลัพธ์ทางคลินิก ด้านจำนวนวันนอนโรงพยาบาล อัตราการตาย การติดเชื้อทางเดินปัสสาวะ การเกิดปอดอักเสบ และการจัดการด้านโภชนาการ ไม่มีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ ระหว่างกลุ่มก่อนและหลังการทดลองใช้รูปแบบที่พัฒนาขึ้น (p > .05) ส่วนความพึงพอใจของญาติหลังการทดลองใช้รูปแบบ สูงกว่าก่อนการทดลองใช้รูปแบบที่พัฒนาขึ้นอย่างมีนัยสำคัญทางสถิติ (p < .01) ผลลัพธ์ด้านกระบวนการ พบว่า พยาบาลมีการดำเนินการเพื่อป้องกันการติดเชื้อทางเดินปัสสาวะ ป้องกันการเกิดปอดอักเสบ และจัดการด้านโภชนาการมากกว่าก่อนการทดลอง อย่างมีนัยสำคัญทางสถิติ (p < .01) ความพึงพอใจของพยาบาลหลังการทดลองสูงกว่าก่อนการทดลองใช้รูปแบบที่พัฒนาขึ้น อย่างมีนัยสำคัญทางสถิติ (p < .01) ทั้งนี้ควรมีการศึกษาติดตามระยะยาว และสนับสนุนให้มีการพัฒนารูปแบบนี้ในผู้ป่วยกลุ่มอื่นต่อไป
