ภาระงานพยาบาลและการจัดสรรอัตรากำลังของห้องคลอดในสถานการณ์ที่เปลี่ยนแปลง

บทคัดย่อ
การวิจัยแบบผสมผสานนี้ มีวัตถุประสงค์เพื่อ 1) วิเคราะห์ภาระงาน 2) ศึกษาปัจจัยที่สัมพันธ์กับการรับรู้ภาระงาน และ 3) ศึกษามุมมองต่อภาระงานและการทำงาน ของพยาบาลและผู้ช่วยพยาบาลที่ปฏิบัติงานในห้องคลอด กลุ่มตัวอย่าง เป็นพยาบาล 24 ราย และผู้ช่วยพยาบาล 10 ราย ตามเกณฑ์การคัดเข้า เก็บข้อมูลเชิงปริมาณโดยใช้ แบบบันทึกข้อมูลการรับบริการคลอดบุตร แบบสอบถามข้อมูลส่วนบุคคล และแบบสอบถามการรับรู้ภาระงาน (The Questionnaire on the Experience and Evaluation of Work: QEEW) ข้อมูลเชิงคุณภาพ ศึกษาจากกลุ่มตัวอย่าง 13 รายโดยใช้แบบสัมภาษณ์การรับรู้และผลจากภาระงานต่อการเป็นอยู่ วิเคราะห์ข้อมูลเชิงปริมาณด้วยสถิติเชิงพรรณนา สัมประสิทธิ์สหสัมพันธ์แบบเพียร์สัน สัมประสิทธิ์สหสัมพันธ์สเปียร์แมนแรงค์ ฟิชเชอร์และไคสแควร์ วิเคราะห์ข้อมูลเชิงคุณภาพด้วยวิธีการวิเคราะห์เนื้อหา ผลการศึกษา พบว่า ภาระงาน ส่วนใหญ่เกี่ยวข้องกับการผ่าตัดคลอด (ร้อยละ 77.70) ซึ่งต้องการอัตราส่วนพยาบาลต่อผู้ป่วย 2.5:1 ผู้ปฏิบัติงานรับรู้ว่า มีภาระมากที่สุดในด้านความเร่งด่วน รองลงมาเป็นภาระทางด้านร่างกาย และด้านปริมาณงานตามลำดับ และ ปัจจัยด้านอายุของกลุ่มตัวอย่างมีความสัมพันธ์กับการรับรู้ภาระงานโดยรวม (r = .43, p <.05) มุมมองต่อภาระงานและความเป็นอยู่ ของกลุ่มตัวอย่าง พบว่า มีแก่นสาระ ใน 3 ประเด็น คือ 1) เหนื่อยและท้อต่อภาระงานที่มากเกิน 2) คนน้อย งานมาก ต้องวิ่งไปมา และ 3) แม้มีปัญหาจากภาระงาน ต้องจัดการให้ได้ ผลการศึกษาแสดงถึง การรับรู้ถึงภาระงานที่มากเกินกว่าอัตรากำลังตามเกณฑ์ จึงควรมีการจัดสรรอัตรากำลังในห้องคลอดที่เหมาะสม
