บทคัดย่อ
การพัฒนาคุณภาพบริการการดูแลต่อเนื่องในผู้ป่วยสูงอายุที่มีภาวะหัวใจล้มเหลวได้รับการยอมรับว่าเป็นมาตรการสำคัญในการลดอัตราการตาย อัตราการกลับมาโรงพยาบาลซ้ำ และค่าใช้จ่าย แบบจำลองการดูแลระยะเปลี่ยนผ่านเป็นกระบวนการพยาบาลที่ต่อเนื่องและครอบคลุม มีเป้าหมายเพื่อให้ผู้ป่วยและญาติผู้ดูแลสามารถข้ามผ่านระยะการเจ็บป่วยไปสู่ระยะการฟื้นหายได้อย่างราบรื่น บทความนี้มีวัตถุประสงค์เพื่ออธิบายแนวคิดและองค์ประกอบของแบบจำลอง รวมทั้งการประยุกต์ใช้ในการดูแลผู้ป่วยสูงอายุกลุ่มนี้ด้วยโปรแกรมการพยาบาลที่พัฒนาขึ้น พยาบาลผู้ดูแลระยะเปลี่ยนผ่านมีบทบาทหลักในการสร้างแผนการดูแลระยะเปลี่ยนผ่านเป็นรายกรณี ประกอบด้วย 9 กิจกรรมการพยาบาลและการดูแล 2 ระยะ ได้แก่ ระยะรับรักษาในโรงพยาบาล และระยะหลังจำหน่ายออกจากโรงพยาบาล ทั้งนี้ เพื่อสามารถตอบสนองความต้องการของผู้รับบริการอย่างได้ผล
