การประเมินคุณภาพของแบบคัดกรองความต้องการการดูแลแบบประคับประคองสำหรับผู้ป่วยที่เข้ารับการรักษาในหอผู้ป่วยหนัก ฉบับภาษาไทย: การศึกษาย้อนหลัง

บทคัดย่อ
การศึกษาแบบย้อนหลังนี้ มีวัตถุประสงค์เพื่อประเมินคุณภาพในด้านความไว ความจำเพาะ และความตรงเชิงพยากรณ์ของแบบคัดกรองความต้องการการดูแลแบบประคับประคองสำหรับผู้ป่วยที่เข้ารักษาในหอผู้ป่วยหนัก ฉบับภาษาไทย ศึกษาข้อมูลจากการสุ่มเวชระเบียนผู้ป่วยผู้ใหญ่ 207 คน ที่เข้ารักษาในหอผู้ป่วยหนักโรงพยาบาลเอกชนแห่งหนึ่ง ระหว่างเดือนมกราคม พ.ศ. 2561 ถึงเดือนมิถุนายน พ.ศ. 2562 แบบคัดกรอง ฯ ได้รับอนุญาตจากผู้พัฒนาเครื่องมือและผ่านกระบวนการแปลย้อนกลับ ทดสอบความเที่ยงแบบสอดคล้องกันระหว่างผู้ประเมิน 2 คน ได้ค่าสัมประสิทธิ์แคปปา 1.00 วิเคราะห์ข้อมูลด้านประชากรและการเสียชีวิตภายใน 6 เดือนด้วยสถิติบรรยาย วิเคราะห์ความไวและความจำเพาะและคะแนนจุดตัดที่ดีที่สุดที่สามารถบอกโอกาสของความต้องการการดูแลแบบประคับประคองก่อนการเสียชีวิต 6 เดือน ด้วยกราฟแสดงคุณลักษณะพื้นที่ใต้โค้ง และวิเคราะห์ความตรงเชิงพยากรณ์ของแบบคัดกรอง ฯ ด้วยสถิติถดถอยโลจิสติค ผลการศึกษาพบว่า ร้อยละ 11.1 ของผู้ป่วยเสียชีวิตภายใน 6 เดือน มีคะแนนจากแบบคัดกรองความต้องการการดูแลแบบประคับประคอง 0-10 โดยจุดตัด เท่ากับ 4 เป็นคะแนนที่ดีที่สุดที่สามารถบอกโอกาสความต้องการการดูแลแบบประคับประคอง มีความไว 0.826 และความจำเพาะ 0.908 และมีพื้นที่การทำนายถูกต้องร้อยละ 92.8 ในด้านความตรงเชิงพยากรณ์ ผลการวิเคราะห์โลจิสติกเมื่อควบคุมอิทธิพลจากปัจจัยด้านเพศและอายุ พบว่าคะแนนจากแบบคัดกรอง ฯ ที่เพิ่มขึ้นทำนายผลลัพธ์การเสียชีวิตได้ 2.1 เท่าอย่างมีนัยสำคัญทางสถิติ ควรมีการนำแบบคัดกรองความต้องการการดูแลแบบประคับประคองสำหรับผู้ป่วยที่เข้ารับการรักษาในหอผู้ป่วยหนัก ฉบับภาษาไทยมาใช้ในการคัดกรองและวางแผนให้การดูแลแบบประคับประคองแก่ผู้ป่วยและครอบครัวต่อไป
