การพัฒนาและการใช้แนวปฏิบัติการพยาบาลทางคลินิกสำหรับการจัดการภาวะช็อกจากการสูญเสียสารน้ำในร่างกาย ในผู้ป่วยวิกฤต

บทคัดย่อ
การวิจัยดำเนินการนี้มีวัตถุประสงค์เพื่อพัฒนาและประเมินผลการใช้แนวปฏิบัติการพยาบาลทางคลินิกสำหรับการจัดการภาวะช็อกจากการสูญเสียสารน้ำในร่างกายในผู้ป่วยวิกฤต ผู้ร่วมวิจัยคือ 1) ผู้ป่วยวิกฤตศัลยกรรมทุกรายที่มีอายุ 18 ปีขึ้นไปที่เข้ารักษาในหอผู้ป่วยหนัก แบ่งเป็นกลุ่มที่เข้ารักษาก่อนใช้และขณะใช้แนวปฏิบัติ 51 และ 45 คน 2) พยาบาลผู้ใช้แนวปฏิบัติ 14 คน วิธีดำเนินวิจัย 2 ระยะ คือ 1) การพัฒนาแนวปฏิบัติ และ 2) การใช้แนวปฏิบัติและประเมินผล ใช้กรอบแนวคิดของสภาวิจัยการแพทย์และสุขภาพแห่งชาติ ประเทศออสเตรเลีย ผลการวิจัยพบว่า แนวปฏิบัติที่พัฒนาขึ้น ประกอบด้วย 6 หมวด คือ 1) การพิทักษ์สิทธิผู้ป่วยและจริยธรรม 2) การให้ความรู้แก่พยาบาลเกี่ยวกับภาวะช็อกจากการสูญเสียสารน้ำในร่างกาย 3) การดูแลผู้ป่วยเพื่อป้องกันภาวะช็อกจากการสูญเสียสารน้ำในร่างกาย 4) การดูแลจัดการผู้ป่วยที่มีภาวะช็อกจากการสูญเสียสารน้ำในร่างกาย 5) การดูแลผู้ป่วยอย่างต่อเนื่อง 6) การพัฒนาคุณภาพการบริการ และมีการประเมินคุณภาพโดยเครื่องมือ AGREE II อยู่ในเกณฑ์ดีทุกด้าน (ร้อยละ 80.55 ถึง 91.66) มีการประเมินความเป็นไปได้ในการใช้แนวปฏิบัติครอบคลุมทั้ง 6 หมวด อยู่ในระดับมาก (ร้อยละ 85.70 ถึง 92.90) ส่วนการใช้แนวปฏิบัติฯ พบว่ากลุ่มขณะใช้แนวปฏิบัติได้รับการประเมิน การดูแลป้องกัน และการจัดการภาวะช็อกสูงกว่ากลุ่มก่อนใช้แนวปฏิบัติทุกด้าน ผลการศึกษานี้แสดงให้เห็นว่าแนวปฏิบัติที่พัฒนาขึ้นมีความเหมาะสมสำหรับพยาบาลในการนำไปใช้เพื่อเพิ่มคุณภาพการดูแลและจัดการภาวะช็อกจากการสูญเสียสารน้ำในร่างกายในผู้ป่วยวิกฤตได้
