บทคัดย่อ
บทค้ดย่อ: การวิจัยเชิงพัฒนานี้มีวัดถุประสงค์ เพื่อพัฒนากรอบสมรรถนะพยาบาลเฉพาะทางที่ดูแลผู้ป่วยหัวใจล้มเหลว โรงพยาบาลมหาราชนครเชียงใหม่ ประกอบด้วย 2 ขั้นตอน 1) ดำเนินการพัฒนากรอบสมรรถะพยาบาลเฉพาะทางที่ดูแลผู้ป่วยหัวใจล้มเหลว ประยุกด์ใช้ขั้นตอนการพัฒนากรอบสมรรถนะของมาเรลลี่และคณะ 2) ตรวจสอบความตรงของเนื้อหากรอบสมรรถนะที่พัฒนา จากผู้ทรงคุณวุฒิจำนวน 5 ท่าน เครื่องมือที่ใช้ในกาวิจัย แบบสัมภาษณ์ผู้เชี่ยวชาญและกรอบสมรรถนะพยาบาลเฉพาะทางที่ดูแลผู้ป่วยหัวใจล้มเหลวฉบับร่างโดยการสัมภาษณ์ผู้เชี่ยวชาญและทบทวนวรรณกรรม ผลการวิจัยพบว่าได้กรอบสมรรถนะทั้งหมด 12 ด้าน โดยมีพฤติกรรมบ่งชี้สมรรถนะ 138ข้อได้แก่ สมรรถนะ 1 ด้านการซักประวัติผู้ป่วยเพื่อประเมินปัญหา 2) ด้านการประเมินสัญญาณชีพ อาการและอาการแสดงผู้ป่วย ประเมินระบบหัวใจและหลอดเลือด ระบบการหายใจ 3) ด้านการดูแลและจัดการปัญหาตามอาการผู้ป่วย 4) ด้านการประเมินปัญหาด้านจิตใจ อารมณ์ สังคมและจิตวิญญาณ 5) ด้นการดูแลผู้ป่วยให้ได้รับการตรวจวินิจฉัยตามมาตรฐานอย่างเหมาะสม 5) ด้านความรู้เกี่ยวกับภาวะหัวใจล้มเหลว 7) ด้านพื้นฐานทักษะการปฏิบัติงานด้านการช่วยฟื้นคืนชีพขั้นสูง 8) ด้านการดูแลผู้ป่วยให้ได้รับการรักษาการปรับยาอย่างเหมาะสม 9) ด้านการให้คำแนะนำผู้ป่วยให้มีความรู้สามารถดูแลดนเองได้ 10) ด้านการดูแลผู้ป่วยที่ต้องใส่อุปกรณ์อิเล็กทรอนิกส์ไฟฟ้าหัวใจ 1 1) ด้านการดูแลผู้ป่วยเมื่อต้องทำการรักษาด้วยการปลูกถ่ายหัวใจและการดูแลผู้ป่วยที่ใช้เครื่องพยุงการทำงานของหัวใจเบื้องต้น และ 12) ด้านการดูแลผู้ป่วยในระ ะยะสุดท้ายของชีวิต ดังนั้นควรนำกรอบสมรรถนะนี้มาเป็นเครื่องมือในการประเมินสมรรถนะพยาบาลที่ดูแลผู้ป่วยหัวใจล้มเหลวเพื่อจัดทำแผนพัฒนาความเชี่ยวชาญต่อไป
