การพัฒนารูปแบบทีมการพยาบาลเพื่อป้องกันการเลื่อนหลุดของท่อช่วยหายใจโดยไม่ได้วางแผนในหอผู้ป่วยหนักศัลยกรรม

บทคัดย่อ
บทคัดย่อ: การวิจัยเชิงปฏิบัติการแบบมีส่วนร่วมนี้มีวัตถุประสงค์ เพื่อพัฒนารูปแบบทีมการพยาบาล และประเมินผลของการใช้รูปแบบทีมการพยาบาลเพื่อป้องกันการเลื่อนหลุดของท่อช่วยหายใจ กลุ่มตัวอย่างคือ พยาบาลที่ปฏิบัติงานในหอผู้ป่วยหนักศัลยกรรม จำนวน 56 คน และผู้ป่วยใส่ท่อช่วยหายใจทางปากที่เข้ารักษาในหอผู้ป่วยหนักศัลยกรรมเดือนกุมภาพันธ์ ถึงกันยายน พ.ศ. 2566 จำนวน 200 ราย เก็บข้อมูลโดยการสนทนากลุ่ม และสำรวจแบบบันทึกอุบัติการณ์ในรายงานผู้ป่วยวิเคราะห์ข้อมูลโดยแจกแจงความถี่ ค่าร้อยละ และวิเคราะห์เชิงเนื้อหาผลการศึกษา พบว่า รูปแบบทีมการพยาบาลเพื่อป้องกันการเลื่อนหลุดของท่อช่วยหายใจ ประกอบด้วย 1) บทบาทและความรับผิดชอบของทีมพยาบาล 2) การฝึกอบรมสำหรับส่งเสริมการเรียนรู้และการพัฒนาทักษะแบบทีม 3) การนำภาวะผู้นำมาปฏิบัติในทีมพยาบาล 4) การตรวจสอบซ้ำการปฏิบัติ 5) การรายงานและประเมินผู้ป่วยข้างเตียง 6) การเยี่ยมตรวจผู้ป่วยร่วมกับทีมแพทย์ 7) การสังเกตและเสนอแนะข้อมูลย้อนกลับซึ่งกันและกัน และ 8) การชมเชยบุคลากรที่ปฏิบัติเป็นแบบอย่างที่ดี ส่วนความคิดเห็นของพยาบาลต่อรูปแบบทีมการพยาบาลที่พัฒนา พบว่า เป็นประโยชน์ และช่วยให้พยาบาลมีความรับผิดชอบเพิ่มขึ้น ด้วยการสนับสนุนของทีมการพยาบาลและการมีส่วนร่วมกับทีมสหสาขาวิชาชีพ ทำให้อุบัติการณ์การเลื่อนหลุดของท่อช่วยหายใจลดลงจาก 2.75 ครั้งต่อ 1000 วัน เป็น 0 ครั้งต่อ 1000 วันใส่เครื่องช่วยหายใจ ดังนั้น รูปแบบทีมการพยาบาลนี้สามารถนำใช้เป็นแนวทางในการป้องกันการเลื่อนหลุดของท่อช่วยหายใจในหอผู้ป่วยหนักศัลยกรรมได้ จึงควรสนับสนุนการใช้ให้เกิดความสำเร็จอย่างยั่งยืน
