A การพัฒนารูปแบบการปรับเปลี่ยนพฤติกรรมสุขภาพของประชากรกลุ่มเสี่ยงในการป้องกันโรคไม่ติดต่อเรื้อรังโดยชุมชนมีส่วนร่วม

บทคัดย่อ

บทคัดย่อ: การวิจัยเชิงปฏิบัติการแบบมีส่วนร่วม มีวัตถุประสงค์เพื่อพัฒนารูปแบบการปรับเปลี่ยนพฤติกรรมสุขภาพโดยชุมชนมีส่วนร่วมสำหรับประชากรกลุ่มเสียงโรค NCDs และประเมินประสิทธิผลของรูปแบบดังกล่าว ดำเนินการ 2 ระยะ; 1) พัฒนารูปแบบ กลุ่มตัวอย่างจำนวน 20 ราย และ 2) ทดสอบประสิทธิผล กลุ่มตัวอย่างจำนวน 50 ราย เครื่องมือวิจัย ได้แก่ แนวทางการสัมภาษณ์ แบบวัดความรู้และ พฤติกรรมการป้องกันโรคไม่ติดต่อเรื้อรัง ตรวจสอบความตรงเชิงเนื้อหาโดยผู้ทรงคุณวุฒิ ค่า CVI 0.91-0.95 ค่าความเชื่อมั่น KR-20 = 0.82, Cronbach's alpha = 0.88-0.91 วิเคราะห์ข้อมูลด้วยวิธี Content Analysis สถิติเชิงพรรณนา และ สถิติ Paired t-test ผลการวิจัยพบว่า รูปแบบที่พัฒนาขึ้นขับเคลื่อนผ่านกระบวนการ "PROMISE" ประกอบด้วย 5 องค์ประกอบหลัก; P-Positive reinforcement, O-Optimum การสร้างตระหนักรู้ในปัญหายอมรับในสุขภาพของตัวเอง ส่งเสริมให้เกิด self-care, R-Result-based management, I-Individual or Clieny Center ส่งเสริมให้เกิด self-regulation, M-Motivation, S-Self-esteem, E-Empowerent ส่งเสริมให้เกิด self-efbcacy ภายใต้กลไกการมีส่วนร่วมอย่างเป็นระบบของชมชน ภายหลังการเข้าร่วมกิจกรรม กลุ่มตัวอย่างมีคะแนนเฉลี่ยด้านความรู้เพิ่มขึ้นอย่างมีนัยสำคัญทางสถิติ (t= 19.30.30, p<.001) และมีคะแนนเฉลี่ยด้านพฤติกรรมการป้องกันโรคเพิ่มขึ้นในทุกด้าน (1 5.97-7.71, p<.001) สำหรับผลลัพธ์ทางคลินิกพบความเปลี่ยนแปลงในทิศทางที่ดีขึ้นอย่างมีนัยสำคัญ ได้แก่ ความดันโลหิตลดลง (t = 3.45, p .05) น้ำหนักตัวและเส้นรอบเอวลดลง (t = 4.36 - 4.46, p <.001) ระดับน้ำตาลในเลือดลดลง (t = 7.09,p< < .05) และดับน้ำตาลสะสม (HbA1c) ลดลงอย่างมีนัยสำคัญ (t = 5.65, p < .01) รูปแบบที่พัฒนาขึ้นสามารถใช้เป็นต้นแบบในการจัดการสุขภาพโดยชุมชนเพื่อการปรับเปลี่ยนพฤติกรรมสุขภาพอย่างยั่งยืน
