Objective : To study clinical data, influencing factors in predicting children with moderate to severe head injury at Nakhonpathom hospital. This study may be useful to prevent children with moderate to severe head injury from death, disabilities and handicaps.

Design : Retrospective descriptive study.

Materials and Method : Retrospective study were done by reviewing medical records of children with moderate to severe head injury who admitted in Nakhonpathom hospital since 1 August 2008 to 31 July 2010. Divided patients into 2 groups by severity of head injury, the first one was mild head injury (GCS 14-15) and the other was moderate to severe head injury (GCS 3-13). Each group was studied in sex, range of age, causes, CT brain and associated injuries. All data were assessed to look for predicting factors in moderate to severe head injury.

Result : Overall children in this study were 476 (male 299, female 177). 70 children (male 45, female 25) with moderate to severe head injury (14.7%), whereas 406 children (male 254, female 152) with mild head injury. CT brain were performed in all children with moderate to severe head injury. All children with moderate to severe head injury had associated injuries. Influencing factors for predicting children with moderate to severe head injury at Nakhonpathom hospital were older children (10.1 - 15 years old.) ( p-value = 0.000) and driving of motorcycles (p-value = 0.000).

Conclusion : Older children (10.1 - 15 years old.) who drived motorcycles were influencing factors in predicting children with moderate to severe head injury, whereas fallings were the most causes of mild head injury of children at Nakhonpathom hospital.
