ประสบการณ์การผ่าตัดนิ่วในถุงน้ำดีผ่านกล้องวีดิทัศน์ 60 รายแรก ในโรงพยาบาลสมเด็จพระพุทธเลิศหล้า

บทคัดย่อ

Objective: to evaluate results of laparoscopic cholecystectomy (LC) in Somdejpraputhlertla hospital

Material and method: A retrospect study was performed in first 60 patients who underwent
laparoscopic cholecystectomy during January 2005 to September 2010. The demographic data, indications for surgery, operative time, results, complications and risk factors for conversion were evaluated.

Results: Most of the patients were female (73.3%). Age affected was mainly in fifth decade of life (33.3%) with an age range of 24-84 years (mean 52.16 ± 14.5). Indications for surgery were symptomatic gall stone (83.3%), chronic cholecystitis (15%) and acute cholecystitis (1.7%). Forty seven cases (78.3%) were successfully performed LC. Two cases (3.3%) had complications from bile duct injuries. The operative time ranged from 60 to 185 minutes (mean 112 ± 35.1). The hospital length of stay ranged from 3 to 5 days (mean 4.2 ± 0.6). Factors related to conversion were skill of surgeon and history of inflammation of gallbladder.

Conclusions: Laparoscopic cholecystectomy is a safe and feasible procedure. In learning phase, the conversion was depended on the experience and previous history of inflammation of gallbladder.
