บทคัดย่อ

     This is a retrospective study of perforated peptic ulcer. This study aimed to find out the incidence, risk factors and management of perforated peptic ulcer. The medical records were reviewed during January 2009 to December 2010. One hundred and one patients diagnosed perforated peptic ulcer were studied with age between 15 to 91 years (average 53 years). There were 82 males and 19 females. Seventy four patients (73.27%) had 1 to 2 risk factors of perforated peptic ulcer. Smoking (66.34%) alcohol drinking (28.71%) and NSAID (21.78%) were the most common risk factors of perforated peptic ulcer, respectively. The underlying diseases of 21 patients (20.97%) were hypertension and chronic obstructive pulmonary diseases. Eighty seven patients (86.14%) had no complications. Sepsis (5.94%) was the most common complication and acute myocardial infarction was the most common cause of death. The prepylorus (67.33%) of stomach was the most common site of perforated peptic ulcer. One hundred patients (99.01%) underwent simple suture and omentum patch. Seventy nine patients (78.22%) had length of hospital stay less than 7 days. Ninety six patients (95.05%) were improved, otherwise five patients (4.95%) were died.
