ผลของการใช้โปรแกรมการสอนระงับปวดด้วยเครื่อง PCA ต่อความรู้ความปวดในผู้ป่วยผ่าตัดเปลี่ยนข้อเข่าเทียมในโรงพยาบาลนครปฐม

บทคัดย่อ

     Total knee arthroplasty (TKA) is one of major operation in orthopedic surgery. It has long and deep incision and causes moderate to severe pain in post operative period. The pain management in TKA with patient control analgesia (PCA) can relieve post operative pain. The objective was to study result of teaching program in using PCA equipment on pain controlled and knowledge of the patients in TKA surgery. The samples were 40 patients undergoing TKA surgery. We divided into two groups, 20 - patients for controlled group and 20 patients for study group. Study equipments consisted of 1) PCA teaching program with VCD. 2) fundamental record. 3) pretest and post test of patient knowledge record. 4) numerical pain scale for pain evaluation. 5) record of frequency of pushing PCA button and amout of analgesic in the first 48 hours post operative. 6) record of complication of analgesic. Data was analyzed by using descriptive statistics, chi-square, Fissher’s exact probability test and independent t-test. Results: Knowledge of patients before teaching program PCA was not different. Knowledge of patients after teaching program PCA in study group was better than controlled group significantly, p-value = 0.001. Pain score of the study group was less than. controlled group in first 24 hours and 48 hours significantly, p-value = 0.001. The frequency of pushing the PCA button in study group was ower than controlled group significantly, p-value = 0.011. The amout of analgesic requirement was lower in study group than controlled group significantly, p-value = 0.001. Conclusion: This study showed the teaching program PCA could reduced pain score, amout of analgesic drug, number of pushing on the button for the patients who underwent TKA surgery. The knowledge of pain management was also better in study group. We suggested that this teaching program can be used widely for post-operative pain controlled in our hospital.
