อัตราการติดเชื้อเอชไอวีจากมารดาสู่ทารก จากการใช้ยาสูตร Zidovudine และสูตร Nevirapine ร่วมกับ Zidovudine ในโรงพยาบาลดำเนินสะดวก

บทคัดย่อ

     Objective : To evaluate the results of AZT compare with combined NVP and AZT regimens on rate of mother-to-child HIV transmission and adverse effect from drug and delivery.

     Design : A retrospective study

     Materials and method : On the basis of government program for the prevention of Mother-to-child HIV transmission, it is implied to practice in Domnuensaduak Hospital in 2 periods. Period I : AZT monotherapy regimen had been given to HIV infected pregnants at 34 weeks gestation until intrapratum period and to infant. ln period II : AZT combined with single dose NVP regimen had been given to HIV-infected pregnants women at 28 weeks gestation until intrapratum period and to infant. The infants had been follow up until were diagnosed as HIV-infected by positive anti-HIV at 12-18 months old.

     Results : A total of 51 HIV infected pregnants having their babies born in Domnuensaduak Hospital. Forty infected pregnants were received Antiretroviral therapy (ART) during antenatal care period (78.43%), 18 infected pregnants were recieved AZT monotherapy, 22 infected pregnants were received AZT combined with NVP. Rate of mother to child HIV transmission among AZT monotherapy and combined AZT  with NVP was the same (0%). Transmission rate among no ART was 9.09%. Common side effect from AZT  was anemia (33.33%), 1 case of fetal demise.

     Conclusion : Although mother-to-child HIV transmission rate among AZT and NVP combined with

AZT was not difference, but combined NVP with AZT regimen is more effective and no serious complication. The most important is to encourage infected pregnants to know about benefit of taking ART and following up regularly.
