บทคัดย่อ

     A retrospective study of 213 pregnant women who had singleton breech presentation, ≥ 28 weeks of gestation and delivered at Banpong Hospital during October 1, 2003 to September 30, 2006. The medical records of these women were reviewed to determine the incidence of vaginal breech delivery and evaluate factors that affected vaginal breech delivery and compare maternal and neonatal complications between vaginal breech delivery and cesarean delivery groups. The incidence of vaginal breech delivery was 40.2%. Multiple logistic regression analysis demonstrated that umbers of previous delivery (OR = 2.948, 95% CI 1.604- 5.419) and more cervical dilatation (OR = 1.444, 95% CI 1.159 -1.799) increased chance of vaginal breech delivery. On the other hand, antenatal care at non - government medical center (OR = 0.4150, 95% CI 0.053- 0.426 ) and estimate more fetal weight (OR = 0.994, 95% Cl 0.992 - 0.996 ) decreased chance of vaginal breech delivery. Neonatal outcomes including lower Apgar score at 1st minute (not for 5th minute), birth trauma, prematurity and admission to the NICU were higher in vaginal delivery. The maternal complications were not different between two groups.
