บทคัดย่อ

          A Retrospective study of patients who were diagnosed as acute cholecystitis at Dumnoensaduak hospital from July 1999 to July 2004. Total number of patients were 158. Number of male patients were 67 (42.4%), female patients were 91 (57.6%). The patients were divided in two groups. First group 125 patients (79.11%) were treated by early cholecystectomy and second group 33 patients (20.89%) were treated by conservative management (delayed choleoystectomy). To compare early with delayed cholecystectomy. Mortality, morbidity and bile duct injury were 0.8 %, 16.8% and 0.8%, respectively, for early cholecystectomy and 3.03%,12.12% and 3.03%, respectively, for delayed cholecystectomy. About 12.12% of patients referred to delayed surgery fail to respond to conservative management. Total hospital stay (mean ± S.D.) was shorter in early group (11.79 ± 6.01 days vs 15.33 ± 3.12). In conclusion : Early cholecystectomy does not carry a higher risk of mortality and morbidity compared to delayed cholecystectomy and should be the preferred surgical approach for patients with acute cholecystitis.
