บทคัดย่อ

Ultrasonography is widely used in regional and general hospital. Its use to apply the ultrasound probe in operative field is recently suggested to get more information for operative decision and detection of surgical complication immediately from this real time imaging.

Intraoperative ultrasonography (IOUS) was performed in 15 patients undergoing surgery for multiple field of operation in Makarak hospital from October 1993 until 1995. 10 cases of hepatobiliary surgery, 3 cases of pancreatic surgery, 1 case of neurosurgery and 1 case of urosurgery.

The study gave more information in 85% and beneficial for surgical strategy in 70%. IOUS is proved to be an accurate assessment of retained common bile duct stone instead of intraoperative cholangiography which is not aviable in all hospital. It give more information and guidance for deep lesion in the liver or brain and useful detection of small and nonpalpable lesion of pancreas. It also provide accurate assessment of extension of lesion and its vascular relation. Thus, operative strategies may be precedes tailored on the basis of information, allowing rational resection where appropriate, while futile attempts at removal of inoperable lesions may be averted.
