Systemic Antibiotics Plus Subcutaneous Drainage in Complicated Appendicitis : A Prospective Controlled Trial.

บทคัดย่อ

          Appendicectomy was performed on 50 patients with complicated appendicitis through a Lanz incision. All patients received systemic gentamicin, chloramphenicol and ampicillin which started preoperatively and continued postoperatively for 3 days. At operation, patients were allocated randomly to receive either no local antiseptic therapy (group A) or subcutaneous drainage, in which 10% povidoneiodine soaked cotton free gauze wicks was placed between each skin suture and brought down to the fascia. The wicks were changed every day and removed on postoperative day 5 (group B). Postoperative wound infection occurred in 8(32%) of the 25 patients in group A and in 1 (4%) of the 25 patients in group B. These differences were statistically significant. No adverse reaction was noted. The conclusion of this study is that systemic antibiotic plus subcutaneous drainage reduced postoperative wound infection.
