บทคัดย่อ

The objective of this study was to evaluate the drug counseling and monitoring service for outpatients focusing on the product of service, the readiness of pharmacists to service and the screening of the patient to receive the service. The samples were 2,140 patients, received the service during November 1993-October 1995. Data were collected from patient's drug counseling and monitoring records and daily service records. The result revealed that :

  1. Pharmacists could identify 507 patient's drug related problems. The most common problems were noncompliance, adverse drug reactions and drug interactions 41.81%, 29.19% and 11.44%, respectively. Pharmacists solved the problems by patient education 85.41% correcting the medication errors 5.52% and consultation with the physicians 9.07%. Among 225 problems which pharmacists could monitor the results of problem resolution, we found that 68.88% of the problems were disappeared.
  1. The number of days which pharmacists could service was 84.09% of the official days.
  2. The average number of patients screened by dispensing pharmacist to receive the service was 5.01 patients/day.

From these findings, the pharmacists should open the service every official days and screen more patients to receive the service.
