บทคัดย่อ

          Background: Pregnancy in teenage has been considered as a high risk situation that increased adverse obstetric and perinatal outcomes when compares with normal age pregnancy. Samutsakhon Hospital has a high incidence of teenage pregnancies and needs more information for managing this group in the aspects of antenatal care, delivery, neonatal outcome.

          Objective: The study aimed to determine the incidence of teenage pregnant women and compare obstetric and perinatal outcomes between teenage and adult pregnant women.

          Setting: Department of Obstetrics and Gynecology. Samutsakhon Hospital, Thailand. Study design: Retrospective control study.

          Method: The study performed analyses of 13,408 pregnant women 10-19 (study group) and 20-34 years old (control group) who delivered during October 1st, 2010 to September 30th, 2012. The data were retrieved from the electronic delivery center program included characteristic, obstetric, neonatal outcomes and analyzed by using relative risk with 95% confidence interval.

          The results: The study group and control group consisted of 1,669 and 11,739 women respectively. This incidence of teenage pregnant women was 12.5% (in all age group) The study group had inadequate antenatal care than control group (RR = 1.52; 95% CI = 1.40-1.65) The study group was associated with anemia (RR = 1.89; 95% CI = 1.71-2.08) preterm labor (RR = 1.75; 95% CI = 1.51-2.01) normal delivery (RR = 1.16; 95% CI = 1.12-1.19) low birth weight (RR = 1.57; 95%Cl = 1.38-1.79) Apgar score at 1 min 7 (RR = 1.59; 95% CI = 1.25-2.03) Apgar score at 5 min s 7 (RR = 2.64; 95% CI = 1.84-3.79) but lower risk of cesarean section (RR = 0.71; 95% CI = 0.64-0.79) operative vaginal delivery (V/E) (RR = 0.47; 95% CI = 0.35-0.62).

          Conclusion: The incidence of teenage pregnant woman was 12.5%. This group had a higher incidence of inadequate antenatal care and increased the risk of adverse obstetric and perinatal outcomes in anemia, preterm labor, low birth weight and low Apgar score.
