Abstract

This study is descriptive research. The objective is to study the situation of managing health risk factors from alcohol, tobacco and substance abuse in the community according to the conceptual framework of innovation chronic care. The research type is descriptive research. The population used in the study is those involved in the management of risk factors in the community 5 sub-district of Ban Na San District, Surat Thani province 33 people. by purposive sampling selection. The instrument used in the study was a semi-structured interview. Data were collected using face to face interview. The results showed that. There are no areas in all 5 sub-districts that have risk management activities carried out. Complete all the components according to the conceptual framework. When divided into 3 levels, found that. Micro levels, Configuration that every area does not carry out are the preparation for participation in the activities of the target group. Meso level, A component that many areas do not implement is support and empowerment through the leadership of health care organizations. And macro levels, The component that many areas do not operate is the support of the legal framework. And building cooperation There are two components that every area can do, namely the promotion and support of leaders. And promoting financing. The results of this study can bring information about the said situation to the development. And create risk management model based on the concept of innovative chronic care management. To be a policy proposal for the community to apply.
