ความเป็นมา: ภาวะแทรกซ้อนของระบบทางเดินอาหารภายหลังการผ่าตัดคลอด ได้แก่ ภาวะลำไส้อุดตันจากกลไกทางกายภาพ (mechanical bowel obstruction) และภาวะลำไส้ไม่เคลื่อนไหวหลังผ่าตัด (postoperative ileus) พบไม่บ่อย แต่ส่งผลต่อการฟื้นตัวของมารดาและอาจก่อให้เกิดภาวะแทรกซ้อนรุนแรง โดยเฉพาะภาวะลำไส้อุดตันจากกลไกทางกายภาพ ทั้งสองภาวะมีอาการคล้ายคลึงกัน เช่น ปวดท้อง ท้องอืด คลื่นไส้ อาเจียน และไม่ผายลมหรือไม่ถ่ายอุจจาระ แต่มีพยาธิสรีรวิทยาและแนวทางการรักษาแตกต่างกันการประเมินอย่างเป็นระบบและการตรวจวินิจฉัยด้วยภาพทางรังสีจึงมีความสำคัญต่อการวินิจฉัยแยกโรค
วัตถุประสงค์: เพื่อนำเสนอและเปรียบเทียบลักษณะทางคลินิก การวินิจฉัย การรักษา และการพยาบาลผู้ป่วยหลังผ่าตัดคลอดที่มีภาวะลำไส้อุดตันจากกลไกทางกายภาพและภาวะลำไส้ไม่เคลื่อนไหวหลังผ่าตัด
การนำเสนอกรณีศึกษา: นำเสนอผู้ป่วยหญิงหลังผ่าตัดคลอดจำนวน 2 ราย โดยรายแรกมีภาวะลำไส้เล็กอุดตันจากกลไกทางกายภาพ และรายที่สองมีภาวะลำไส้ไม่เคลื่อนไหวหลังผ่าตัด วิเคราะห์เปรียบเทียบข้อมูลทางคลินิก ผลการตรวจทางห้องปฏิบัติการ การตรวจวินิจฉัยด้วยภาพทางรังสี การรักษา และการพยาบาล
ผลการศึกษา: ระยะเวลาการเริ่มมีอาการ ปัจจัยเสี่ยง ลักษณะอาการทางคลินิก และผลการตรวจเอกซเรย์คอมพิวเตอร์ช่องท้อง (computed tomography; CT) ช่วยแยกความแตกต่างระหว่างสองภาวะและนำไปสู่การวางแผนการรักษาและการพยาบาลที่เหมาะสม กรณีศึกษาสะท้อนความสำคัญของบทบาทเชิงรุกของพยาบาลในการประเมินอาการตั้งแต่ระยะเริ่มต้น การเฝ้าระวังภาวะแทรกซ้อน การดูแลแบบองค์รวม
และการประสานทีมสหสาขาวิชาชีพ
สรุป: ภาวะลำไส้อุดตันจากกลไกทางกายภาพและภาวะลำไส้ไม่เคลื่อนไหวหลังผ่าตัดมีอาการคล้ายคลึงกัน แต่ต้องใช้แนวทางการวินิจฉัยและการรักษาที่แตกต่างกัน การประเมินอย่างครอบคลุม
การวินิจฉัยที่รวดเร็ว และการพยาบาลตามหลักฐานเชิงประจักษ์ มีความสำคัญต่อการลดภาวะแทรกซ้อน ส่งเสริมการฟื้นตัว และเพิ่มความปลอดภัยของมารดาหลังผ่าตัด
