การปลูกถ่ายตับเป็นมาตรฐานของการรักษาผู้ป่วยโรคตับระยะสุดท้าย ขณะเดียวกันก็เป็นหัตถการที่มีความเสี่ยงต่อภาวะแทรกซ้อนสูงทั้งในระหว่างการผ่าตัดและภายหลังผ่าตัด มีการเปลี่ยนแปลงของระบบการทำงานต่าง ๆ อย่างรวดเร็ว การดูแลผู้ป่วยปลูกถ่ายตับในระยะแรกถือเป็นระยะวิกฤต ผู้ป่วย ต้องได้รับการดูแลในหอผู้ป่วยวิกฤต ตรวจติดตามและคงไว้ซึ่งการทำงานของหัวใจและระบบทางเดินหายใจ ประเมินผลการทำงานของตับและภาวะแทรกซ้อนอย่างทันท่วงที
