การประเมินสัญญาณเริ่มต้นของภาวะหายใจล้มเหลวระหว่างการใช้อุปกรณ์ให้ออกซิเจนอัตราไหลสูงผ่านสายสวนจมูกในผู้ป่วยเด็ก: การทบทวนวรรณกรรมแบบกำหนดขอบเขต

ภาวะล้มเหลวของการรักษาด้วยอุปกรณ์ให้ออกซิเจนอัตราไหลสูงพบได้ร้อยละ 10–30 ในผู้ป่วยเด็ก การล่าช้าในการยกระดับการรักษาส่งผลต่ออัตราการเจ็บป่วยและเสียชีวิต ปัจจุบันยังขาดการจัดระบบองค์ความรู้ด้านสัญญาณเตือนเริ่มต้นและเกณฑ์มาตรฐานสำหรับการประเมินทางการพยาบาล วัตถุประสงค์ เพื่อ (1) จัดหมวดหมู่สัญญาณเริ่มต้นที่ทำนายภาวะล้มเหลวดังกล่าวในผู้ป่วยเด็ก และ (2) ระบุเครื่องมือ คะแนนทางคลินิก และตัวชี้วัดทางสรีรวิทยาที่ใช้ประเมินสัญญาณเหล่านั้น การทบทวนวรรณกรรมแบบกำหนดขอบเขตตามกรอบ Joanna Briggs Institute รายงานตามแนวทาง PRISMA-ScR สืบค้นเดือนธันวาคม พ.ศ. 2568 จาก PubMed (MEDLINE), CINAHL (EBSCOhost) และ SCOPUS ครอบคลุมงานวิจัย ค.ศ. 2015–2025 ใช้กรอบ Population–Concept–Context ผู้วิจัยสองท่านคัดกรองอย่างอิสระ ประเมินความสอดคล้องด้วย Cohen's Kappa
จากบทความที่ค้นพบ 156 รายการ ผ่านเกณฑ์คัดเข้า 16 บทความ ครอบคลุมผู้ป่วยเด็กอายุ 0–18 ปี ในหอผู้ป่วยวิกฤต หอผู้ป่วยเด็กสามัญ และแผนกฉุกเฉิน อัตราภาวะล้มเหลวอยู่ที่ร้อยละ 11–23 ส่วนใหญ่เกิดภายใน 24 ชั่วโมงแรก สัญญาณเตือนจำแนกได้ 4 กลุ่ม ได้แก่ ตัวชี้วัดการแลกเปลี่ยนก๊าซ (SpO₂/FiO₂ ratio และ ROX index ที่ไม่ดีขึ้นใน 1–4 ชั่วโมงแรก) การตอบสนองของสัญญาณชีพ (อัตราการเต้นของหัวใจและอัตราการหายใจลดลงน้อยกว่าร้อยละ 20 ใน 1–2 ชั่วโมงแรก) คะแนนความรุนแรงทางคลินิก (CRS, Wood–Downes–Ferrés และ p-HACOR) และปัจจัยเสี่ยงพื้นฐาน (อายุน้อยกว่า 3 เดือน คลอดก่อนกำหนด โรคหัวใจพิการแต่กำเนิด พยาธิสภาพนอกระบบทางเดินหายใจ และ FiO₂ มากกว่า 0.6) เครื่องมือที่มีประสิทธิภาพสูงสุด คือ p-HACOR score ที่ 1 ชั่วโมง (AUC = 0.951) ROX index (AUC = 0.716–0.90) และ Clinical Respiratory Score ≥ 6 ที่ 12 ชั่วโมง (AUC > 0.90)
ภาวะล้มเหลวของการรักษาสามารถคาดการณ์ได้ตั้งแต่ระยะเริ่มต้น ผ่านการบูรณาการสัญญาณเตือน 4 กลุ่ม โดยเฉพาะในช่วง 1–2 ชั่วโมงแรก ผลการศึกษาสนับสนุนการพัฒนาแนวทางการประเมินทางการพยาบาลที่เป็นมาตรฐาน
