Study of stroke fast track management procedure for patients admitting at Lamphun hospital

This study aims to evaluate the management procedure of Stroke Service Plan in Lamphun hospital. Data were collected from 1 October 2014 to 30 September 2016. Results found that, a total of 55 acute ischemic stroke patients were treated with rtPA and admitted in ICU Stroke unit. In 2015, the first year data, 72% (16/22) of the patients receiving the rTPA had a better clinical condition (mRS 0-3 score). The waiting time for PT, PTT INR and CBC indicated 54.3 minutes which was significantly longer than a standardize time of 45 minutes (p<0.001). The CT brain process took 34.9 minutes which is significant longer than standard of 30 minutes (P-Value = 0.026). In order to have immediately solve this problem
a multidisciplinary team have provided the PT, PTT and INR machine to be installed at the ER as well as using LINE application to make as soon as possible notification the CT brain result. The assessment of the second year after improving the processes revealed that the success rate had increased to 84% and the mortality rate had reduced less than 7% which was satisfied the KPI of the Regional of Health Service Provider.
