THE MINIMUM EFFECTIVE DOSE OF DEXAMETHASONE FOR PREVENT POST OPERATIVE NAUSEA AND VOMITING (PONV) AFTER GYNAECOLOGICAL SURGERY

Nausea and Vomiting are side effect, found in 30% of post-operative patients. These symptoms effect patient’s discomforts and contentment. Dexamethasone is and effective drug for prevention of post-operative nausea and vomiting. But the lowest dosage for treatment of such symptoms is controversy. This Randomized controlled trial study aims to evaluate the lowest dosage of Dexamethasone for prevention of post-operative nausea and vomiting in patient undergo gynecologic surgery at Lamphun hospital, Thailand. Seventy-two patients were randomly enrolled in the study, divided in 3 groups by randomized envelop seal. Group 1 was control placebo (2 ml. normal saline), group 2 received 2 mg. (0.5 ml.) of Dexamethasone in 1.5 ml normal saline and group 3 received Dexamethasone 4 mg (ml) in 1.0 ml. normal saline. Patients were given Dexamethasone before anesthesia. Post-operative pain was treated by giving opioids in first 2 hour. Pain score was assessed by Wong-Baker FACES pain Rating Scale. Nausea and Vomiting and Treatment of symptoms were assessed 24 hour, post-operatively. General characteristic data was not different in all groups. The lowest dosage of Dexamethasone to prevent post-operative nausea and vomiting (PONV) is 4 mg. (P=0.039) Summary 4 mg. Dexamethasone can prevent post-operative nausea and vomiting in patients undergo gynecologic surgery.
